Confused about Testogel / Testotop dosage – Doctor said once daily, prescription says 1/3

Hey guys, posting this for a friend who doesn’t speak English.

He recently had a blood test and his testosterone came back at 2.18 ng/mL (7.56 nmol/L) which seems pretty low.

He then had an online consultation with a doctor. During the consultation, the doctor told him to use Testotop testosterone gel 125mg once a day for 90 days.

After the consultation, he received an order confirmation from the pharmacy and it suddenly says 1/3 of a dose per day. He didn't receive a prescription because the doctor sent it straight to the pharmacy. Nothing was mentioned about using only one third of a dose during the consultation.

We’re assuming this might simply be a misunderstanding, because if the doctor intended him to use only 1/3, wouldn’t that normally have been discussed during the consultation? He would also need some way of measuring exactly the same amount every day. Otherwise, how would you consistently measure 1/3? Would you need a precision scale or something similar?

There’s another thing that makes this confusing. The package leaflet says that the gel shouldn’t be kept and used the next day because the alcohol can evaporate after opening, which can affect the product.

So if he is supposed to use only 1/3 of a dose, what is he actually supposed to do with the remaining 2/3?

He already contacted the pharmacy about this, but they said they can’t advise him and that he needs to clarify it with the doctor. Unfortunately, doctor consultations through the service aren’t available again until next week.

We’re just trying to understand whether using 1/3 like this is even something people normally do with Testotop, or whether this sounds like a prescription/communication error. We're not sure if 1/3 of it will even make a difference.

Has anyone here used Testotop or a similar testosterone gel and experienced something like this?

Thanks!

The standard starting dose for Testotop 2.5% transdermal T gel is 2.5 gram sachet (62.5 mg T).

It is available in 2.5 gram sachets which contains 2.5 grams gel (62.5 mg T) or a 5 gram sachet with contains 5 grams gel (125 mg T).

If the doctor's prescription was for 125 mg T/day then it should be the full 5 gram sachet.

If the pharmacy stated that the dose is 1/3rd which seems too low for a starting dose then you need to contact the doctor and get this straightened out.

Keep in mind men using big pharma Androgel 1% strength the starting dose is 5 grams gel which contains 50 mg T and most men will not achieve a high enough TT and more importantly FT on the starting dose.

Many men need the higher-end dose 10 grams gel (100 mg T) per day in order to achieve a healthy or high TT/FT level.

Look over the peak serum T concentration achieved 2 hrs post-dose using 100 mg transdermal T gel (Testotop®)!

Figure 1 | (A). Study 1. Sex hormone testosterone and cortisol concentrations following transdermal T and placebo across 5 time-points and 16 subjects. Application of 100-mg transdermal T increased total serum T levels significantly relative to placebo and pretreatment as early as 90-min posttreatment. Serum T increased gradually to a maximum observed concentration 2h posttreatment. Testosterone levels in the placebo group (dashed black line) remained constant throughout the timely progression of the experiment. Serum C concentrations decreased with time independent of transdermal T, according to cortisol's independent circadian rhythm. Cortisol levels did not differ significantly between transdermal T and placebo treatment. (B). Study 2. Sex hormone testosterone and cortisol concentrations following transdermal T and placebo across 3 time-points and 19 subjects. Serum T concentrations were significantly elevated 2h posttreatment. Although significantly higher relative to baseline and placebo treatment, 255-min posttreatment, serum T levels gradually decreased compared to 2h posttreatment. Serum C levels decreased independent of transdermal T. Whiskers indicate 99% confidence intervals. Red-filled and black dots represent the observed scores for the T and placebo treatments, respectively.
