My Total Testosterone went up from 344 to 474, but I have high estradiol and low FT

I would not say your E2 is high and it looks like it may be the standard instead of the sensitive test as well - which most often overstates your true E2 level. Your T:E ratio is 17, which is in the ideal range.

What are your symptoms?

Fatigue, slight gyno(had since I was a teen), unable to loose to get under 23% body fat, anxiety, trouble focusing, low confidence, feeling weak, low mood.

I wouldn't suppress your estradiol (certainly not on the basis of the incorrect test this doctor ordered). I would consider a second opinion with a TRT specialist. A second opinion doesn't commit you to treatment with that doctor, but might offer you ideas not touched on as of yet.

I discovered there is now an OTC SERM, or at least a product entitled "Rebirth" that has SERM like properties. Someone logged themselves using it on another forum and there Total test went from 62 to in the 400 range after 2 months. Their estradiol increased a little. they were also using another OTC product though, so I'm not sure how well Rebirth is on it's own and how it'll effect Free Test in addition to total. Some guys might be against using OTC products, but I like to have a backup option. I'm not saying I won't go see a TRT doctor, but there is only one that isn't over 50 miles away and I don't own a car and thus need to get rides. The anti-aging doctor that is the only specialist close charges $300 for test injections and offers $650 pellets. I can still see him for a free consultation, but it's not very promising from the sounds of it.

I would give Defy Medical a call and consider a consult with them. They are far less expensive than that anti-aging Doc and you don't need to drive there as they work via phone and email. Many of us use and recommend them.

Also, as an FYI - Pellets are literally the worst possible choice for TRT and the better docs refuse to even use them anymore. They are decades out of date.

Yeah, I know Defy's pricing and with the blood tests, they aren't affordable. If I decide to get a part-time job while in college, I MAY be able to afford them them but that has to wait until after my next foot surgery. That's why I was wondering about OTC products and I don't want to go the "research chemical" route. I know, pellets are junk. I was just telling you guys about what he charges and that basically I was offered pellets up front without even going in. I think he gets the secretaries to try and push product before you even go in, that's what it seemed like at least.

Could I use a SERM to increase my Free Testosterone, along with my Total T? If I used a SERM, would I have to use an AI as well?

Were you diagnosed with true gynecomastia, or are you dealing with a fat distribution issue? Your estradiol, though measured with the incorrect test, isn't high and suppressing it with an AI can lead to a world of misery.

Yeah, I wasn't going to use an AI unless a SERM increased my estradiol, which I read can happen, I don't know if that just happens when one cycles off a SERM, or it happens while taking a SERM regardless. Honestly, no doctor has ever check even though I have mentioned it to multiple doctors. Actually one doctor did check when I was a teen and told me I had some breast tissue. Well, that tissue has never gone away. It is less noticeable when my body fat is down, but it's still there. The OTC SERM I mentioned earlier is supposed to be similar to Nolva, but supposedly doesn't cause any rebound *shrugs*. It's either try that, or order Nolva.

Forgot to add, I haven't been able to get lower than 22% bodyfat in 6 years. It doesn't matter how I track my diet, or how much cardio I do.
