mL to units: reading your syringe​


The single most common dosing question is how mL maps to the units marked on an insulin syringe. On any U-100 syringe the scale is fixed: 1.0 mL is 100 units, regardless of whether the barrel is a 0.3, 0.5, or 1 mL syringe.

mg values use the standard 50 mg/mL carnitine and choline concentration. Confirm your own vial, since compounders vary.

Lipo-C dosage per day​


Most protocols are two or three times a week, not daily. Some users prefer a daily 1 mL subcutaneous dose to hold carnitine and B-vitamin levels steadier and to smooth out the energy effect. Daily use is a preference, not a requirement, and it burns through a vial about three times faster. If you go daily, keep the single dose at 1 mL SQ, rotate sites, and use the morning so the B-vitamin stimulation does not affect sleep. There is no reported advantage to daily dosing over 2–3× weekly for fat loss specifically; the case for daily is energy consistency.

Lipo-C dosage chart for women​


The formulation does not change by sex. What differs in practice is starting volume and titration speed, since women in these programs often start at a lower body weight and report more injection-site sensitivity at the top of the range.

Most women in the community threads land on 50 units twice weekly and adjust from there. Pregnancy and breastfeeding are stopping points: concentrated methyl-donor combinations are not well studied in pregnancy, and guidance is to avoid Lipo-C unless a clinician judges maternal benefit to outweigh theoretical risk under obstetric supervision.

What is Lipo-C?​


Lipo-C is a compounded injectable that combines lipotropic agents (compounds that move fat out of the liver) with amino acids and cofactors that support fat metabolism and energy. It is prescription-only and compounded by pharmacies including Empower Pharmacy and Defy Medical. "Lipotropic" means fat-moving: these compounds help keep fat from accumulating in the liver, support bile production, and improve how efficiently stored fat is converted to energy.

Lipo-B vs Lipo-C. Empower's Lipo-B (MIC) is methionine, choline, and B12. Lipo-C drops the standalone B12 focus and adds L-carnitine, the fatty-acid transporter, making it the broader-spectrum metabolic formulation and the more commonly prescribed of the two for fat-loss support. Formulas differ by compounder, so the exact contents on your label are what matter.

Ingredients and what each does​


Standard Empower Lipo-C: Methionine 15 mg/mL, Choline chloride 50 mg/mL, L-Carnitine 50 mg/mL, Dexpanthenol (B5) 5 mg/mL, in water for injection. Some compounders add inositol, B12, thiamine, or pyridoxine.

Titration: how to ramp​


Weeks 1–2: start at 0.5–1 mL, 1–2× per week. Watch tolerance: odor, injection-site reactions, GI changes.
Weeks 3–6: if tolerated, move to 1 mL, 2–3× per week. This is where most users settle.
Week 6+: if energy or fat-loss response is minimal, try 2 mL IM 2× per week, or daily 1 mL SQ.
Reassess at 12–16 weeks: if there is no measurable or subjective benefit, reconsider whether Lipo-C is the right tool for you.

How to inject: sites, needles, technique​


Subcutaneous (SQ). Use a 1/2-inch insulin syringe, 28–31g. Best sites are pinched abdominal fat and outer thigh. Inject at 45 degrees into the pinched fold. Keep SQ volume at or below 1 mL per site, since larger volumes leave firm, slow-absorbing lumps.

Intramuscular (IM). Use a 5/8-inch, 25–26g needle. Best sites are the vastus lateralis (outer thigh), ventroglute, or dorsoglute. IM suits volumes above 1 mL and absorbs faster. Aspirate slightly to confirm you are not in a vessel before injecting.

The odor effect. Choline and carnitine breakdown, plus any added B12, can produce a sulfur or "fishy" smell in sweat, urine, or breath within hours of a shot. It is harmless and usually fades within a day or two. Intensity varies by formulation and individual metabolism.

Stacking: B12, GLP-1s, and TRT​


Lipo-C + B12. Some compounders build B12 into the formula, others supply it separately. If yours does not include B12, a separate 1–2× weekly B12 injection is worth considering for energy. Many members report B12 as the component they feel most directly.

Lipo-C + GLP-1 agonists (semaglutide, tirzepatide). This is the fastest-growing use case. GLP-1s drive aggressive caloric restriction, which accelerates lean-muscle loss and can impair liver fat clearance. Lipo-C is co-prescribed to hold L-carnitine levels during that deficit, support hepatic fat mobilization as lipid flux rises, and partly offset GLP-1 fatigue through B-vitamin and carnitine support. No pharmacokinetic conflict is known; the two are usually injected at different times of day. Confirm the combination with your provider.

Lipo-C + TRT. TRT improves insulin sensitivity and lipid metabolism but does not directly clear hepatic fat. For men on TRT in a fat-loss phase, Lipo-C adds complementary support, and L-carnitine has independent literature backing for improving lipid profiles in men on testosterone.

Side effects and storage​


Most reactions are mild: injection-site soreness, warmth, or redness that fades quickly, and the transient odor effect above. Uncommon but real: GI upset, and rare hypersensitivity. In one retrospective series of over 2,000 compounded lipotropic injections, systemic hypersensitivity ran below 0.1 percent. Absolute contraindications include hypersensitivity to any component, severe hepatic failure, and sulfur-amino-acid disorders such as homocystinuria. Renal or hepatic impairment calls for extra caution, since carnitine clears renally and methionine loads the liver.

Storage: refrigerate at 36–46°F, keep away from light, do not freeze. Most compounded vials carry a 28–30 day beyond-use date after dispensing.

Does Lipo-C actually work?​


It depends what you ask it to do. Each component (L-carnitine, choline, methionine) has legitimate mechanistic data behind it for fat metabolism and liver function. In meta-analyses, L-carnitine produces modest but statistically real reductions in body weight, and it does that by moving fatty acids into mitochondria for oxidation. That effect only shows up when there is a calorie deficit and activity for it to act on. Members who report the clearest results pair Lipo-C with a deficit and training, or run it alongside GLP-1 therapy. Standalone results without diet change are modest at best.

I am actually not sure myself. I use 1.5 ml twice per week currently due to some griping I saw about pain. (minimizing injections).

I use the Defy sent .25 gauge Sub-q (5/8) needles. Since I switched to a .27 gauge needle for T-injections I figured I'd get use out of those needles to prevent the surface pain some have described.

I don't know that I feel that much different yet I've had mine about 3 weeks?
 
I have used this in the past and could never see or feel anything from it. I know its not a 'magic shot' for instant weight loss or energy, but still...never felt anything...perhaps its just me.
 
I use Lipo-C 5-7 times per week. Dose is 1ml, using a 29g 1/2" insulin syringe. When I first started I was doing SubQ in stomach and sides. This made me "lumpy" and was uncomfortable. I switched to upper glutes, shallow IM, and it's been great.

I just started, so time will tell on the weight loss. I'm aggressively dieting right now, and I know the biggest factor is "calories in vs. calories out" - but if this can boost my energy, accelerate some fat loss, and help preserve lean tissue - it's worth a short term investment to me...then I'll re-evaluate if I want to continue when I get to "maintenance."
 
I think I may have already told you before, but I'm pinning 1mL EOD into my quads. I've pinned this nutrient in every possible location, and this is the result:

Abdomen - burned like hell going in, and I can't take more than .5mLs without it leaving a lump.

Deltoids - burned like hell going in, 1mL left a big lump, and my shoulder was sore for 3 days.

Glutes - no burn or any PIP. The only problem is the difficulty pinning myself in this area.

Quads - no burn or PIP. Easiest location for me to pin. This is my "go to" spot.

The Lipo-C seems to give me a blast of energy post injection. Injecting very slowly seems to be the best technique to combat the burn.
 
Question for you guys. Any ideas on Half-Life of the key elements in Lipo-C?

I've seen dosing frequencies of daily, EOD, and 3x/week. Defy told me I can inject daily or 2-3 times per week in various amounts (1-3ml) as long as I don't exceed 9ml/week (calculated from 3 times per week at 3ml/injection).

Trying to figure out if I should do smaller daily injections or larger but less frequent injections.

Any comments?

FP,

You're going to have to turn yourself into a lab rat, and find out what works best for you. Personally, I can't imagine pinning 3mLs of fluid in a single injection. That is a lot of fluid, and you're not going to be able to do that much SubQ.

I know it's a pain in the ass, but more frequency and less volume seems to work best (at least for me). I don't want to inject deep IM, and that's what you're going to have to do if you're thinking of injecting a large amount like 3mLs. Let us know how it pans out.
 
I did 1ml twice a week in the beginning, subq insulin pin in the belly, honestly didn't feel much of anything. Another downside is that your whole body reeks of B vitamins for a day after the shot.
 
Thanks guys. Interesting how we are all different, and I have no problem being a lab rat in that respect.

Here are some things I've tried:
- Tried injecting 1mm in multiple spots and had no issues in quads or belly subQ. I could feel a little bump for a while in the belly, probably due to 1mm being a lot of liquid, but no pain.
- Tried 2mm dose one day (2 separate 1mm injections - 1 in each quad) - and seemed to feel more of an energy boost that day. (Not sure if placebo effect though).
- Tried a week of daily 1mm shots for a week. Felt pretty good all week. (Not sure if placebo, and plus so many other things are in play, how can you really isolate?)
- Never noticed any smell or changes in body odor.

FWIW, YMMV, and all that. :)

I'll likely continue with probably near-daily injections of 1mm, since I believe in the ingredients. But after a couple months (after I've lost some more body fat) I'm likely going to taper off and stop and see if I notice any difference.
 
I just started taking Lipo-C from Defy early last week. I pin 1ml EOD and I can say I've noticed a slight increase in energy a few hours after taking it (like Fireproof, not sure if placebo affect as well). I may try the 2ml as separate pins on separate quads (I've only pinned quads to this point) to see if that does more for me. I may also switch to daily pins, to see if that helps as well. So far, I've lost about 5 lbs since originally starting it, but I'm not sure how much of that is related to TCyp, my changed diet, exercise, or Lipo-C. I do know, however, that this combination is working for me and I'm feeling really good, so I plan on continuing to take it but will see what dose and frequency works best for me.

I notice the odor, but for me I only smell it on my hands and spot where I pin so I wash my hands thoroughly after I'm done. I actually prepared my injections on my ironing board one time next to a couch in my room. I must have dropped a bit because I can still smell it from that time over a week and a half ago. The first day I took it, I did notice a strange mineral/vitamin taste in my mouth. But I have not noticed it since.

I'll post my findings once I figure out what has the best effect.
 
I just started on lipo-c today, 1 ml 2x a week. I was surprised at how much 1 ml really is! I injected it sub-q into my abdomen and didn't have any issues. Y'all are definitely right about the smell, it is pretty potent. I'm definitely looking forward to seeing any changes.
 
I'm hoping for weight loss benefit too - I'd be interested in hearing if others had this effect. Just started today and was planning 1ml 3x week.
