Have tried every protocol under the sun. No Libido, ED, good labs

Most here have some very good suggestions and point, What sucks about TRT is it is different for everyone. It's not as easy as taking 400mg ibuprofen and some of your pain goes away. But in regards to E2 level like @FunkOdyssey stated E2 can be high, but for me I feel best in the 50-80pg range(I also have lower SHBG around 20). Any lower I get libido issues. For me to accomplish this I have to micro dose injectable estradiol. HCG raises my E2 a lot, but it also raises my progesterone and prolactin I found.
Can you remind us your protocol with details on doses, frequency, TT/FT and SHBG?

Currently at 16mg Test E daily along with 0.1mg estradiol valerate EOD.

Labs from this April
TT 838ng
FT was 255pg (35-155pg)
SHBG was 21
E2 was 68
DHT was 154ng (12-65 ng/dL)

I just speculate that I need higher E2 because my DHT is high from TRT alone. I tried injectable DHT E and felt like trash pretty quick. So my theory is my DHT somehow counters my E2 leading me to needing higher levels of E2. I have tried lower doses around 10-12mg daily which my DHT will still be in the 90's but I will have TT in the 600's and E2 in 20's (without E2 injections). So for some reason I favor DHT conversion.

I've been on TRT for little over 3 years and have tried every protocol under the sun, without ANY success. I am now in a phase where I realize I might need professional help, so if anyone has any online doctors that they would recommend and are ok with me doing UGL, please let me know.

Regardless, if anyone has any input, please look at my labs below:
  • Hemoglobin = 15.7 g/dL (normal)
  • HbA1c = 4.4% (excellent)
  • Glucose = 84 mg/dL (normal)
  • Creatinine = 1.21 mg/dL (normal)
  • Total Cholesterol = 143 mg/dL (good)
  • HDL = 38 mg/dL (slightly low)
  • LDL = 93 mg/dL (good)
  • Triglycerides = 62 mg/dL (excellent)
  • Vitamin D = 36.76 ng/mL (good)
  • Estradiol = 47.4 pg/mL (high)
  • Total Testosterone = 11.07 ng/mL (very high)
  • Free Testosterone = 65.9 pg/mL (high)
  • LH = <0.07 mUI/mL (very low/suppressed)
  • FSH = 0.32 mUI/mL (very low/suppressed)
  • Prolactin = 7.2 ng/mL (normal)
  • SHBG = 34.5 nmol/L (normal)
  • TSH = 1.79 μUI/mL (normal)
  • PSA = 0.581 ng/mL (good)

This was on 120mg Test E a week split into two doses. It's important to mention that I do not feel any, and I mean any better on Test P daily, lower doses, higher doses, or even 4 month cold turkey quit, at least in regards to Libido and Erections. The only time I ever felt good was the first month of TRT (honeymoon, where I literally felt full of confidence, good EQ, etc.) Now I have no orgasms, soft erections, etc.

I've been on a Dr prescribed TRT for over 10yrs and have learned A LOT. (mostly, the hard way...) First off, you will not find a Doctor ok with you on UGL...

But, for your sake, health and to conclusively come to ANY conclusion, you'll need a product you can absolutely depend on the quality and content of the pharmaceutical (UGL prolly ain't the answer here)
If you are on 60mg x 2 p/week (120mg) and your trough Total T is freaking 1100+ something ain't jibing here! Either it's not 120mg's or you Never needed TRT in the 1st place.

There's and endless list of possibilities for your symptoms. But, let's just concentrate on the things that are in Black and White from your Lab's. Correct them and stay consistent with your home spun TRT treatment or find a reputable doctor to prescribe and get you back on track... Personally, in your shoes, I'd opt to have a real Doc oversee your next 6+ months at the very minimum.

Your Free T is ridiculously high. (Hormone Optimization is about balance.)

Your E2 is def High (for some guys) and hence, if you are one of those guys, it would give you soft erections and possibly No orgasm or "Meh" @ best. Could cause High BP as well and put you Libido in the gutter. Other guys, are fine even @ 60+! (I'm Not one of those guys)

Your D3 is "in Range" but very LOW! -should be about 80 min and 100 optimally.

I'd Love to see your Iron and Ferritin values. Crashed Iron and Ferritin for TRT patients is all too common and totally overlooked by GP's, PCP's and Clinics all day everyday for society in general. Sadly TRT doc's miss this one too. If you aren't showing signs of Anemia on your CBC, the idiots all dismiss ANY possibility of Low iron. When in fact Iron deficiency WITHOUT anemia is at least TWICE as common!!

PS TRT causes Low Ferritin on its own and donations only compound the issue. (its Not only caused by donating as most guys mistakenly think) Most guys will be fortunate to ever get it above 50-60 on TRT and if you take iron, eat a ton of Red meat, etc.. Your Hct and Hb will eventually go through the roof and you'll be donating all your efforts into a bag, to keep your Hct and Hb under control...

Those are the glaringly obvious things to me, that jump out based on your Lab's and your statements.

P.S. Your LH and FSH will be flat lined while on TRT or any exogenous test. That's normal
