Firm, painful knot at injection site

I've been on TRT for 10 years. I inject 0.2 ml (40mg) of testosterone cypionate twice per week. A few months ago I started injecting HCG (200mg) at the same time. I first draw the testosterone into the syringe, and then draw the HCG into the same syringe. I use a new needle when drawing each dose to avoid contamination (2 needles are used for each injection). In the past few weeks I started diluting the HCG (using 5ml of bacteriostatic H20 instead of 1ml) so that it is easier to measure out my dose. My last three injections have all caused me to have a painful hard knot develop at the injection site. This last one is red. I've had a painful knot develop a few times over the years, but never three times in a row.

Any ideas on what's happening here?? These knots go away, but it takes a week or so for each to resolve.
 
I have had the similar experience, in fact just am getting over the same situation but it is not with T. My Dr. had no explanation, I just chalk it up to an instance where the injectable pools and creates a nodule, in time it is absorbed into the body.
