I am 74. Been on TRT for about 7 years. Still working part time as Nurse Practitioner working in mens' health.
I inject 120 mg testo cyp weekly. 1/4 tab anastrozole twice a week (theoretically).

My libido is strong. 10

Erections are a 10. I like tadalafil/sildenafil. Like knowing its got my back. Have morning wood every day.

I'll rate orgasms 8-9. Certainly not as intense as years ago.

I am disappointed in ejaculate volume. Down a bit and thinner in consistency. Seems a bit better since I've stopped uroxatrol. I'm having a Urolift in 2 weeks. Expecting some improvement after that.
I’m surprised u need 0.5mg of an ai per week on just 120mg of test per week. What symptoms did u have on this dose of test prior to using an ai?

Have u ever tried HCG for more ejaculate volume? Here’s a list of things some guy took, and labeled it “the holy grail” for increasing ejaculate volume lol. I saved it incase I ever have issues in that area. And I’ll post a video I saved where another guy talks about what he uses. I think sunflower lecithin and lecithin might be the same tbhbg

I was listening to a podcast where the guy said that he figured out that most of his bipolar patients just had thyroid issues. He said that he was able to “cure completely” which he referred to as they came off all meds and functioned normally again, or he cut down their meds considerabley, in 80% of his bipolar patients, just by optimizing their thyroid function, with either NDT and/ or synthetic T4/T3 replacement.

Also look into the carnivore diet. I’ve heard anecdotally many people with psych disorders, including bipolar, completely get rid of their mental illness by going carnivore and starting to weight train. The weight training part wasn’t mandatory for every one of them, but for most I would say.

And then I would just look into optimizing stress and sleep. Sleep being the biggest one. And take vitamin D if ur not already, take magnesium if ur not already (preferably liquid magnesium chloride), and try to get as much sunlight as u can, even while supplementing with vitamin D

@tropicaldaze1950 Perhaps on this forum it’s uncommon but I believe through lifestyle changes it’s very achievable for a majority of otherwise healthy men.

If you have ever gone through what a lot of men on this forum are going through (I have) you will understand it is a very dark and desperate place to be in and they need to understand there is a way out. Maybe not a raging libido and granite erections but at least a healthy libido and erections that can penetrate and remain hard. Supplements, regular high intensity resistance training , cardio and a well rounded healthy diet can do wonders to the physiology of the human body. Engaging in a TOTAL wellness program (not just TRT/HRT) is quite possibly one of the keys that could unlock the male potential.

Men need hope, they need to hear there is a way out and I hope I can help bring that to some because I know that darkness and it isn’t pretty.
I think u nailed a huge aspect of this that many don’t focus on. Lifestyle/ wellness. Things like eating as healthy as possible, lowering stress, optimizing sleep, weight lifting, etc. I’ve also been weight lifting since the age of 13. Will be 35 this year. Have never taken more than a week or two off. And have always been a health freak. And I’ve never really had erection issues in my life. And even when my libido is low, it’s probably where a lot of guys wish their libido’s were. And same with erections. Even when mine aren’t how I like them to be, they’re probably where a lot of guys wished theirs would be. Not trying to brag or anything, I just think all the lifestyle factors that I optimize and that I’m an overall health freak factors in big time. I’ve tried every protocol u can probably think of, and throughout them all I’ve never had considerable libido or erection or BP issues. Just not ideal libido and erections compared to where I like both to be, which is a 10/10. I attribute these things to eating as close to carnivore as I can (while still eating benign carbs for gym performance, like white rice and select fruits), weight training every week, doing everything I can to optimize sleep, lowering stress the best I can, and supplementing with things I can’t get enough of in my diet, like magnesium, vitamin D, iodine, and I eat raw grassfed beef liver twice per week as my whole food “multivitamin” I think guys would have much different TRT experiences if they optimized their whole body and mind, rather than just optimizing their T levels

I take magnesium, calcium and D, as well as going to the beach several times a week. I've gone through a ton of supplements over the past 16 years. Changed diets many times, too. My wife's a vegetarian and the carnivore diet would flip her out. I eat fish.(salmon, flounder, shrimp) Eggs and dairy are problematic for me. I do miss eating meat and perhaps I have some deficiency(ies) that meat would correct. Going for major labs, today, for my urologist, including iron,TIBC and ferritin. My wife is about to become his patient because her primary won't run a comprehensive thyroid panel nor any other hormone labs. She has symptoms of early stage dementia and neither her primary or neurologist will order extensive labs to rule out thyroid or pernicious anemia. IMO, they'll end up killing her because of their incompetence. I'm angry and sad everyday about her situation.

He’s been on a few very popular podcast recently, like the Joe rogan experience and lex Friedman’s podcast. Think he talks about optimizing sleep right at the beginning of the Joe rogan podcast he did. But to give u the cliff notes, here’s the stack that he says cures some people that have been insomniacs for basically their whole lives. He goes over a bunch of other stuff, like sleep temps and light exposure, but these are the supplements he says have a profound effect on people he helps with sleep issues. Also, get a good pair of red/green light blocking glasses if u haven’t already. They’re a game changer. It’s basically impossible to get actual quality sleep if ur eyes are exposed to artificial blue and green light prior to bed. I recommend blublox. Get the ones that cover the sides of ur eyes as well. Think they’re called “onyx”. Not all red light blocking glasses are equal tho, so go with a quality pair if you’re gonna get some

She just couldn’t function properly. She ended up finding out plant toxins, specifically oxalates, were the issue. She’s carnivore now and thriving. If ur wife wants to get better, she has to give up being a vegetarian, and ideally go as close to carnivore as she can. Another amazing resource is Dr Paul Saladino. I understand that the chances of u converting her are slim to none, but her current diet is slowly killing her unfortunately. And I don’t say that lightly.

And I wouldn’t waste anytime with ur urologist. Just have her sign up with a reputable hormone clinic and she’ll get the care ur looking for. I highly recommend Defy medical, but was watching a video with the guys from Medicial Health Institute of Miami, and they seem really good. Here’s the video I watched

I will look into all that you shared. My wife isn't going to eat meat or fish. Years ago, she used to eat lots of eggs and dairy. I agree that given what is happening to her, family history(sisters) of dementia, coupled with poor diet, she is, right now, on a path to worsening mental and physical decline. Now, I'm trying to save her life and my life. I'm effing exhausted. I have become her brain. Though I will never give up on her, her resistance to change might kill her. I have to be prepared to accept that.

17. There hasn't been much science to support claims that supplements can increase the volume of a man's ejaculation.

Past hydration and holding off from ejaculating for a bit, there isn't much one can do to change the volume of their ejaculation. According to Morgentaler, there isn't much science behind other methods or supplements.

Ur a good man. Unfortunately her diet is what’s killing her the most. It’s so hard to watch people putting in effort to do what they think is best for their health, but they’re actually doing the opposite. Replacing and balancing her hormones will definitely help, but without the proper diet/ vitamin and minerals, as well as all the plant defense chemicals/ toxins wreaking havoc on her body and brain, she’s fighting an extremely uphill battle unfortunately. It’s just unfortunate that we only get one life and because of incorrect information, in regards to diet, she’s choosing to most likely end her life early, and not live those last years of her life healthy and happy. She could easily live a long and healthy life from this point out if she would just make the correct lifestyle choices. But if there ends up being even a slight chance that she’s open minded, in regards to diet, I have a bunch of videos I saved that explain really well why mainly being carnivore is the optimal diet for humans, and videos explaining why plants wreak such havoc on the human body and don’t supply it with what it needs to function properly. Just remember, there’s thousands of videos on YouTube of people explaining why they went from vegetarian/ vegan to carnivore, but u won’t find any videos on carnivores going vegetarian or vegan. It’s because once u start eating a diet the body prefers, it becomes extremely obvious and relatively easy to maintain good health. Plus, once u learn the reasons why being carnivore is healthy for u, such as learning about plant defense mechanisms/ plant toxins, u can’t just unlearn that stuff, and it’s impossible to ignore once u know about it. But good luck with u and ur wife’s health journeys. If she isn’t willing to be open minded, in regards to her diet, all u can do is respect and support her, which is exactly what ur doing, and I definitely commend u for it

I wanted to give some of you long-term users a quick quiz, just so we can develop some neat anecdotal data points from this forum.

Very low. The reason I came to TRT in the first place. TRT never has helped this. But it always has been low even as a young man.

This is difficult to rate with no one else to compare it to. But I would say that I have ED issues that have gotten worse over the years.
6. How is your orgasm strength? Either rate out of 10, or describe in your own words.
No idea how to rate this. Isn't an orgasm and orgasm? Very different that from my wife though who I would say has a long drawn out one with lots of aftershocks. That certainly doesn't describe mine.

Again hard to rate when you have no one else to compare to. Porn stars? Ha! But if I had to guess I would say low volume.

I do the holy grail as well. I seem to be getting juicier having stopped uroxatrol.

It’s just unfortunate that we only get one life and because of incorrect information, in regards to diet, she’s choosing to most likely end her life early, and not live those last years of her life healthy and happy. She could easily live a long and healthy life from this point out if she would just make the correct lifestyle choices. But if there ends up being even a slight chance that she’s open minded, in regards to diet, I have a bunch of videos I saved that explain really well why mainly being carnivore is the optimal diet for humans, and videos explaining why plants wreak such havoc on the human body and don’t supply it with what it needs to function properly. Just remember, there’s thousands of videos on YouTube of people explaining why they went from vegetarian/ vegan to carnivore, but u won’t find any videos on carnivores going vegetarian or vegan. It’s because once u start eating a diet the body prefers, it becomes extremely obvious and relatively easy to maintain good health. Plus, once u learn the reasons why being carnivore is healthy for u, such as learning about plant defense mechanisms/ plant toxins, u can’t just unlearn that stuff, and it’s impossible to ignore once u know about it. But good luck with u and ur wife’s health journeys. If she isn’t willing to be open minded, in regards to her diet, all u can do is respect and support her, which is exactly what ur doing, and I definitely commend u for it
Thanks, man. You're on the money; hormone balancing is only part of the story. One needs a diet that is nutrient dense. There aren't any indigenous societies where they exist solely on a plant diet. They also hunt or fish or both. Maybe my doctor can prevail upon her. Even eggs and dairy, which as I wrote, she used to eat quite a bit of, would be a plus. When we walk on the beach, she goes up to guys fishing and lectures them about how bad fishing is. WTF! I just walk away from her. A few years ago she took me to this great seafood restaurant near us. Been around for decades. She ate oysters, then she told me she did it to please me but she would never eat fish again. Double WTF!!!

So, you see that convincing her to even modify her diet is going up against a brick wall. Generally, every living thing has an inborn will to survive. I don't know if she understands that she's damaging herself by eating a narrow, restricted diet in the name of living, in what she believes, is an ethical way of life. Save the animals and fish and kill yourself in the process. No animal or fish will mourn her death.
