Male, 48. Keep in good shape with regular sports and cardio, lift but not to any serious degree. Diet is balanced. I only supplement Vitamin D and Omega 3 currently. Nightly Tadalafil (5mg), no other medications. I don’t smoke, vape or use drugs and only use small amounts of alcohol approx every 3 months.
Symptoms: low libido (almost non-existent), ED, no morning erections, frequently passing small volumes of urine, area just above pubic bone feels irritated, fat gain to chest and lower abdomen despite no changes in diet, fatigue, difficulty falling asleep and waking 2-3x per night to either urinate or for seemingly no reason, mood quite flat. All deteriorated over last 5 years. Been tested for diabetes - no issues.

Testosterone - 16.9 nmol/L (range 8-29) - "normal"
Free testsoterone - 0.293 nmol/L (range 0.198 - 0.619) - "normal" ***seems low in the range***
FSH - 2.7 iu/L (range 1.5-12.4) "normal"
LH - 5.8 iu/L (range 1.7-8.6) "normal"
SHBG - 42.5 nmol/L (range 18.3-54) "normal" ***seems high in the range***
Free Androgen Index - 39.8 (range 35-92) "normal" ***seems low in the range***
Albumin - 46 g/L (range 44-50) - "optimal"
TSH - 2.49 miu/L (Range 0 - 4.2) - "Optimal"
T4 - 133 nmol/L (range 0 - 300) - "normal" (normal shown as 59-154)
FT4 - 19.1 pmol/L (range 0 - 33) - "normal" (optimal shown as 12-17)
FT3 - 5.2 pmol/L (range 0-11) - "normal" (optimal shown as 3.1-5)
TPOAB - 16.7 kIU/L (range 0-200) - "normal" (no optimal shown)

Others that might be relevant:
Active B12 - 62.6 pmol/L (range 0-165) "possible deficiency" (normal is shown as 70-150)
Folate - 10.5 nmol/L (range 0-49) - "normal" (only just though as normal is shown as 10-30)
Vitamin D - 156 nmol/L (range 0-275) - "normal" (optional shown as lower than I am at 75-120)
Ferritin - 69 ug/L (range 0-1100) - "normal" (normal shown as 30-300)
CRP - 0.87 mg/L (range 0-11) - "optimal"
PSA - 0.73 ug/L (range 0-4) - "normal"
They dont test for Estrogen or Cortisol.
The prolactin looks like it could be an issue?
I think my free testosterone looks quite low and my SHBG looks quite high - could this account for my issues?
Should I be looking at TRT or is there another fix(es) I should be looking at?
