TRT Dosage too High

Hello,

I started TRT 10 weeks ago with 386 ng/dl.
I do 20 units every three days.
I also donate blood and my Hemoglobin (Hgb) is 15.5
My recent lab results show 1464.00 ng/dl testosterone.
My question is: How much shall I lower the injections? 30%, 40%, or 50% to get to around 850 ng/dl?

The most common strength used TC/TE 200 mg/mL.

U-100 insulin syringe

1 ml = 100 units

0.01 mL = 1 unit = 2 mg TC/TE


You are injecting .20 mL (20 units) = 40 mg T every 3 days (93.3 mg T/week).

The standard starting dose is 100 mg T/week or 50 mg every 3.5 days.

Your weekly dose of T is not that high yet you are still hitting a whopping TT 1464 ng/dL.

The s**t kicker here is how many days post-injection were your labs done as we always want to test at the true trough (lowest point) before your next injection.

Seeing as you are injecting every 3 days there is going to be a difference between the peak--->trough as true peak would be achieved within 24 hrs post-injection which means. your trough will be lower.

Top it off that you are missing the most critical blood marker here free testosterone.

Although TT is important to know FT is what truly matters here as it is the unbound fraction responsible for the positive effects.

TT means nothing without knowing where your FT truly sits.

Most men hitting a very high trough TT on doses <100 mg/week would have highish/high SHBG.

Even then if your TT is a whopping 1464 ng/dL and you have high SHBG it is a given that your FT would be high-end/high but again was this at or closer to your peak or true trough as it will make a big difference here on your next move.

What is your SHBG?

Any time the dose is titrated whether going up or coming down 20-25 mg T would be a sensible move on moving your numbers in the right direction.

Again before even considering lowering your dose you need to let me know how many days post-injection your labs were done and we need to find out where your FT sits.

The injection was about 36 hours before the blood work (test).
The lab results do not show any SHBG and no Free Testosterone, despite I told him to do it.
