TRT - 90 Day Bloodwork Opinions? Having some minor issues.

Been on TRT for about 90 days now. Test Cyp, 160mg/wk. I inject 80 mg twice a week, every 3.5 days IM.

I just had my 90 day doctor call to review my bloodwork. (First of all, I’m switching TRT companies.)

The consensus I’ve gotten from other forums is that my test should be higher after 90 days, and E2 is a bit high. Should be about half.

Overall, I love the benefit of TRT. Energy, workouts, etc. I’m having some intermittent ED issues. Not all the time, but my wife notices. I know it could be a number of things, but maybe E2 could be a factor?

Could it be that I’m just a low responder to the test? Maybe it’s just taking a while to build up? I did the blood draw just before injection in the AM.

They prescribed me arimadex, but I haven’t touched it. I’m terrified of the side effects, especially with long term use. Could any number of natural estrogen blockers put a dent in this E2, or is it not even worth my time?

Your Free T doesn't look that high, would help if you provided reference ranges for the Free T. Your issues may not be the levels at all, but the difference between peak and trough if large enough can impact sexual function in certain men.

A lot of men low SHBG need frequent daily or EOD injections. The smaller frequent dosing can have a large impact on the estrogen levels.

I would not worry about your E2 level unless you have obvious high E2 symptoms. I would also recommend dosing more frequently due to your lower SHBG. Stay with 160mg a week and break it up to daily or every other day doses. Wait another 4-5 weeks and re-test levels.
 
Thanks, I will absolutely try this. How do I dose out 160/wk EOD? The math is a bit tricky unless I’m just overthinking it. One week I dose 3 time a week, the next I dose 4. How does one split that up?

To inject daily using an insulin syringe, and assuming your Test is 200mg/ML, draw up to the 12th tick mark. Each tick mark is 2mg, and 24mg x 7 days works out to 168mg per week, which is a tad higher than what you do now. Or, just draw to the 11th tickmark and inject daily for 154mgs per week.

The 160 mg per week corresponds to 160 / 3.5 ~= 46 mg EOD. Your Vermeulen calculated free testosterone is at a healthy young-man average. If the measurements were taken at a pre-injection trough then peak values are potentially 50% higher and above-range. I would reduce the cypionate intake to a total of 126 mg per week, dosed at 36 mg EOD or 18 mg daily.

You should find out if your estradiol was measured with a sensitive, mass spectrometry-based test. If not then there is potential for a positive bias in the result; you need confirmation before accepting that it is this high.

I understand that lab numbers don't equate with how one is feeling & functioning and we are critical of doctors and patients who chase numbers, as if they're the holy grail. That said, @80 mg 2x week, those numbers aren't impressive, particularly free testosterone. Maybe that could be a possible reason for the intermittent ED.
