Starting TRT Hopefully Soon

I am a 50 yr old who discussed TRT with my primary Dr but was abruptly told no after my total T showed 470ng/dL.
I explained to him about my symptoms and he pretty much told me Im just depressed and the symptoms will go away if I lose some body fat and reduce stress.

My labs
Total 470
Albumin-4.2g/dL
Shbg- 38 nMol/L
Free T - 50pg/mL
Bioavailable T - 108 ng/dL
FSH - 4.4 mIU/mL
LH - 1.5 mIU/mL
PSA- 1.5 stable for years
I dont seem to have the male pattern baldness gene.
BP is always 106-110/75-80
BF - 27%
Estradiol -32 pg/mL
Have high visceral fat 3lbs


Lift/cardio4-5 times a week- 150-200 mins cardio
Drink 60-80ozs of water a day.
Males in my family all suffer from metabolic disorder.
Dad had an aggressive form of prostate cancer at 77. He beat it but passed a couple years later from something else.
Males in my family also walk around with normal hematocrit levels 48-50% My 10 yr average is 48%.
I had a leg DVT due to COVID. It was my fault as while working from home recovering from Delta my dumb ass rested my foot directly under my right knee for hours a day while trying to work. That is exactly where the clot was.
Ive had ever clotting disorder done and they are all negative for having any disorders.

I am stating my family history because that was the reason my primary said TRT never going to happen because previous clot and prostate cancer in the family.

New hormone Dr put me on
Compounded 20% gel- 200mg daily because topical less likely to have issues with hematocrit.
Qs
1. This starting dose seems high to me. This a normal starting dose?
2. Should I get bloodwork done more often like every 2 months because I have a higher normal hematocrit level?
3. If I dont see good results with gel, would cream work better?
