Opinions my recent Testosterone lab results

Here are my latest results and wanted to get opinions on it. I have tried everything to naturally get my Testosterone up to its Max before I started trt. This is my absolute best attempt and the highest I have achieved since losing one testicle and chemotherapy due to testicular cancer. Diet, exercise, lots of strength training, keeping stress low, good sleep etc. not on any meds not even blood pressure meds. I was in the 300s for a long time. I have been consistently in the 400s now but I still would like more energy. No sexual issues at all. Definitely not as clear brain thinking as before cancer but that could be chemo too. Libido isn’t great though. I only have one testicle now so please keep that in mind. 6”4 200 lbs mid 40s. I also have PSA, FSH, estradiol, prolactin results if they would be helpful.
Is it worth trying trt to see if it helps with energy and brain fog or is this pretty good and may not notice much difference on trt.

Although TT is important to know FT is what truly matters as it is the active unbound fraction of T responsible for the positive effects.

Were your labs done in the early morning in a fasted state?

When testing TT/FT/BAT you always want to test at the peak which would be in the early morning (within 2 hrs) of waking would be ideal and in a fasted state otherwise your results will be skewed.

Unfortunately your FT was not tested using an accurate assay,

You had it tested through Labcorp using the known to be inaccurate direct immunoassay which can easily underestimate FT and such assay should never be used/relied on.


Age 40-49: refernce range 6.8-21.5 pg/mL

You would need to have your free testosterone tested using the most accurate assay the gold standard Equilibrium Dialysis to see where it truly sits especially in cases of altered SHBG.

Even then looking over your response especially recovering from testicular cancer and T levels hovering mostly in the 300s and sitting in the low 400s as of now chances are your FT is sub-par.

The only way you would be hitting a healthy FT with a not so stellar TT 410 ng/dL would be if you had very low SHBG.

If anything I would retest your FT using an accurate assay to see where it truly sits.

Your best bet and the most cost effective option would be paying out of pocket and using Nelson's discounted labs which offers testing through Quest Diagnostics using the most accurate assays for TT (LC-MS/MS) and FT (Equilibrium Dialysis).

If anything retest your FT using an accurate assay and you would need to get a thorough panel done before even considering jumping on T-therapy!

You are still missing some critical blood markers.

Blood work should be done for TT, FT, estradiol, SHBG, DHT, prolactin, Vit D, DHEA-S, LH/FSH, PSA, full thyroid panel (TSH, Free T3, Free T4, Reverse T3, antibodies), salivary cortisol (Four Specimens), lipids, CMP, CBC, ferritin/iron and CRP.
