Is it time for TRT?

Hello all. 36 years old. Just received my bloods today and as expected low levels all across the board. I had a test 3 years ago which also showed my testosterone levels at 10.9.

I have had a few symptoms for the past few years but always just put it down to working shifts and getting older lol.

I did this test at 11.30am and had a small breakfast bar around 3 hours prior, nothing 12 hrs prior to that.

Suffering with major fatigue, brain fog, short term memory is quite bad at times, struggling to gain muscle like i used to, and a major one for me is a lot of problems with joints/bones in my knees & ankles although not sure if this is correlated to low test.

I see my LH & FSH is also low and me and my wife are trying for our second child from next month, would their be any problems here? Worth mentioning that we had our first child 16 months ago and she fell pregnant on our first attempt.

Right now I am thinking to try for our second child and then possibly look to start TRT. Or is it worth attempting to bring levels up via HCG, Clomid, Tamox etc first?

I had done multiple cycles in my young/mid 20’s but always did an extensive PCT. Maybe this is the result of that.

Thank you for your help.

Total testosterone - 9.70 nmol

Free testosterone - 0.1661

Free androgen index - 26%

FSH - 1.8 IU/L

LH - 1.5 IU/L

Oestradiol - 41.2 pmol

Since you're trying for a child, I would say that it is essential to try Clomid/Enclomiphene or HCG first. Also note that your symptoms (including low T) could be due to things other than hormones, particularly some sort of chronic infection such as lyme disease. You could also have some sort of food sensitivity. Oxalates could cause your joint issues as could infection. So, the risk of adding Clomid/HCG is fairly low in the near term,, but it would be best if you could get a work-up from a holistic/integrative doc who can screen for infections, mold toxicity, and other things that could be at the root of your issues.

You never had your bloodwork done at the proper time which would be in the early am within a few hours of waking up as we want to test at peak let alone you need to be fasted otherwise your results will be skewed!

Even then chances are your T levels will still be subpar.

Keep in mind although TT is important to know FT is what truly matters as it is the active unbound fraction of T responsible for the positive effects.

You need a more thorough set of labs too.

At the very least you should have tested TT, FT, estradiol, SHBG, prolactin, LH/FSH, full thyroid panel, and cortisol.

Any dysfunction thyroid/adrenals can easily mimic low-T symptoms.

All my other marks are within range I will post below. Had a full panel as well and all within range. Folate a little low thats all.

Albumin - 49 g/L

SHBG - 37.3nmol

Prolactin - 170 mlu/L

DHEA - 9.55 umol
