No one has clinically followed free estradiol in any study. Low sex hormone binding globulin may increase not only free testosterone but also free estradiol.

But, as I said, no one has done studies using free estradiol as a variable, so I cannot speculate if a total estradiol of 30 pg/ml should be treated in the presence of low sex hormone binding globulin. My guess is that I would not treat.

If its a non sensitive test then its worthless trying to adjust your protocol based on that number. Check your DHT and maybe try to raise that and see how you feel, but you might be asking for nothing but trouble treating e2 off the standard test.

You might be right, but I've been using them for 6 tests straight and my baseline prior to TRT was 26 with these tests. Are they not consistent? I don't care if they read high, I only care if they read high consistently.

I can tell you I felt better sexually when I had 2 readings done at both 26 and 31. The last two at 72 and 54 I have high E2 symptoms and erection issues.

I do not have access to sensitive tests at the moment. When I return to the States this summer I will, but I can say the results of the ECLIA tests line up with the way I feel. And Doctor John, and many others, treat based on symptoms first according to his own posts.

Hi Nelson. I totally agree with you in this matter. However I have a question : What about the cases with low or very low SHBG ? A Estradios level of 30pg/ml is high enough to use Anastrozol ?

Alex, because I'm considering using Arimidex this is question I wanted answered as well. I think this question/answer from All Things Male might help you:

Guy 1 has a SHBG of 20, guy 2 has an SHBG of 40. Could guy 1 potentially feel the impacts of that estrogen level greater than guy 2? Guy 1 might actually feel high estrogen sides whereas guy 2 is in a sweet spot?

Maybe the more direct question: is the estrogen test #140244 similar to a TT reading in that SHBG isn't factored in yet, or is it similar to a FT reading where SHBG has done its binding and we are seeing the end result?

This doesn't answer your question directly but it shows that symptoms and lab tests don't always lineup and yes, you could feel worse than the next guy with the same E2 reading.

Hi, Vince. I also have low SHBG and I saw that you have your T shots EOD because of that. Please, could you explain the advantage of doing so ? Sorry if maybe you have already done such explanation. tks.

This helps lessen the need for an AI or enables the use of much less AI. You're doing more frequent injections but each injection is less in dosage which helps to moderate conversion to estrogen (E2). A large one time dose (or even twice per week) is simply more than the body can use and some will consequently be aromatized.
The low SHBG simply stated means the body uses it very quickly which has your T going up and down over the course of as little as 3.5Days which is a common dosing schedule. EOD creates a much smoother and consistent ride from injection to injection.

When do you test for estradiol? You mention before treatment, but I've been on it now for 6 weeks. Do I test a few days after my next test shot or how exactly?

Would really appreciate any thoughts on this. If I'm being tested the day before my next shot (6th day), I'm assuming my estradiol levels have come down from my last shot. However, when am I supposed to measure the estradiol to see if I am below where I need to be to take Arimidex in the first place? Do I measure AFTER I take the shot by1 day? 2 days? 3 days? 4 days? 5 days? 6 days? I'm taking my TRT shots weekly. Thank you. If this has been answered before, please point me to the post.

I would take 2 tests. "Peak" test 48 hr after Tcyp shot and "Trough" test right before next shot. This would give a fairly accurate range.

I feel so dumb ? I have been on TRT for 3 yrs and actually self medicating due to Dr lack of even reading how blood different panel readings effect one another and I have not watched my SHBG that closely . I just focus on E2 and Free and total test ! I am setting up BW for next week !

example my Dr. Scripts Astrosonol at 1 mg per day ! I have 4 full bottles of 1mg tabs and one half bottle of .50 broken tabs and I break a half about every 6 days and E2 stays around 26 to 30 and Test around 700 ?

I plan to post BW when I get it back I may be off and not know it ? But I feel good just not that super high I got about 2 years ago and I think most men feel really great the 1st year and then their body kind of settles into the new you ?

But after 3 years I have researched every where and changed protocol and found what looks like good balance from a BW Suggested panel and what was stated on that Panel and it explained why and what BW meant ? I could post if a MOD approved it but GD and Chris Vettester have read the BW panel and how to read instruction I am referring to

Please correct me if I am wrong on anything I state because I have basically read my own BW and had no Dr's assistance ?
