19 Week Blood Draw--Anxiety/Fine Tune

Okay, I am almost fine tuned, but have had bad anxiety and feeling a bit bonkers at 12-36 hours post shot. In consult with Defy, I have been tweaking my Tcyp dose. My 19 week blood test at 36 hours post shot shows, I think, that 80mg Tcyp is too much, as it peaks too high, especially my DHT and E2. I am now at 75mg TCyp/250 HCG @3.5 days.

Question: Does high DHT cause anxiety in the same way E2 can? I know both are high, so it is hard to determine which it is.

80mg TCyp/250HCG/AI 0.20 @3.5 days
7.5 Week Blood Draw at 8:30 AM 14 April 2017 (Trough)
Total Test____1130 ng/dL 348 - 1197
Free ________22.3 pg/mL 6.8 - 21.5
E2 Sensitive __25.5 pg/mL 8.0 - 35.0
I felt a little crazy 12-36 hours post shot, so Defy said try dropping to:

70mg TCyp/250 HCG @3.5 days
13.5 Week Blood Draw at 8:30 AM 19 May 2017 (Trough)
Total Test____675 ng/dL 348 - 1197
Free ________16.5 pg/mL 6.8 - 21.5
E2 Sensitive __25.3 pg/mL 8.0 - 35.0

I did not feel a good on this dose, but anxiety post shot disappeared, so Defy said to try going back up to:
80mg TCyp/250HCG @3.5
This had me feeling bonkers again at 12-36 hours, so I took a blood test at 36 hours to attach numbers to how I was feeling:

19 Week Blood Draw at 8:30 AM 26 June 2017 (36 hour post shot)
Total Test____1895.8 ng/dL 348 - 1197
Free ________39.9 pg/mL 6.8 - 21.5
E2 Sensitive __43.8 pg/mL 8.0 - 35.0

DHT_________136 ng/dl 30-85
TSH_________1.70 uIU/ml 0.45-4.5

Free T3_______3.3 pg/ml 2.0-4.4
DHEA________215 ug/dl 102-416

During your next consult discuss and consider smaller TIW dosing (on T cyp and HCG) to level out the peak/trough a bit more.

Thanks, Dr. Saya. I will be sure to bring it up.

Now I know why I felt so crazy for 72 hours post shot on 160mg Test Cyp/500HCG every 7 days, before I started with Defy; the spikes must have been crazy high.

What is your SHBG at? have you had that pulled at any time in all this?

If you do go to TIW use the same total weekly dose just split it three ways, so 160 / 3 = ~53mg. I would leave HCG at 250 and minimize the variables in your program, change just one thing at a time. 24-36hrs post too me would seem to indicate an E response.

At 7.5 and 13.5 your labs were done day before injection? Surprising that your tt is almost cut in half going from 80mg every 3.5 days to 70mg. At week 19/ 36hrs post injection your tt peak is way too high and your free is almost double, even though your sensitive e2 is slightly over top range your tt is so high you would think it would not cause too much of an issue? Unlikely that too high dht would contribute to anxiety.

I have not pulled it during this run, only prior to my Clomid restart attempt in Jan. '17.

Prior to that I was on once a week shots which started ticking SHBG down. Here are the numbers:
9-16-16=49.6/ range 16.5-55.9
10-31-16=42.6
12-6-16=39.5

So, I should get it pulled next time.

Also, thanks for the tips on TIW. I thought about switching to that anyway, as I do not mind frequent shots.

What's the anxiety about...weepy? emotional? hot in bed? feel like fighting? Short-tempered? Angry?
In order emotional>short-tempered>weepy.

Yes, 7.5 and 13.5 were both trough days.

I agree, that the TT and Free are way too high. Perhaps that affects my neurotransmitters? The T/E ratios is solid, but maybe E2 @43 is too high, for me, no matter the T-level. I will try to even this out and retest.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
