Do you know the difference between free and total testosterone?

Most people don't, but it's an important distinction. Watch this video to learn more about these two measures of testosterone and what they mean.

Is there an 'optimal' amount of free T? Dr. Morgentaler alludes to men who have a decent total T but are still symptomatic(fatigue, no morning erections, low libido, etc). What I've understood is that increasing the dosage of T will lower SHBG and increase free T. I was hoping that Dr. Morgentaler would cite specific numbers in regards to free T. And is it specific to each man, where one might feel great with a free T of 250 pg/mL, but another man needs 350 or 400 or 500(pg/mL), which would translate into a higher dose, thus a supra high total T.

Somewhere south of a bulging purple hypertensive crisis and north of walking dead with no libido, energy or function down below. Unless the individual in question is into either of those options. That is a good question.

Always put units on your numbers.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
