Nelson's house u heard it here first!

Dr. Shalender Bhasin!

* over the past 4 decades he has become a global leader in the biology of androgens, sarcopenia and aging related functional decline, his research has shaped clinical guidelines, its informed public health policy particularly around testosterone therapy, anabolic interventions and then the development of function promoting therapies

Pay attention to 25:40-34:36 of the presentation where he explains the importance of measuring free testosterone using a standardized Equilibrium Dialysis method!

* Elevated haematocrit is common with TTh, typically occurring within 3–12 mo. Levels up to 54% appear safe, but higher values may increase CV risk [15].

* Treatment adjustments are required for haematocrit >54% (requiring withdrawal and phlebotomy in high-risk cases)

* Aside from TRAVERSE, meta-analyses have now concluded that TTh, in doses restoring physiological levels, is not associated with elevated cardiovascular risk.

* One safety consideration that does remain is erythrocytosis. As noted, testosterone often raises hematocrit. In T4DM, for example, about 20% of men on testosterone developed hematocrit >54%, versus 1% on placebo [16]. Similar results were observed in the HEAT-registry [19]. While an increased red cell mass can be beneficial up to a point (alleviating anemia), excessive erythrocytosis...

Thread 'High Hematocrit on TRT: practical “numbers plus action” guide'

Feb 24, 2026

How often should men on TRT check their hematocrit?

The standard monitoring schedule includes a baseline CBC before starting TRT, a recheck at 3 to 6 weeks, another at 3 months, and then at 6 and 12 months once stable. Any dose or formulation change resets the monitoring clock, requiring a recheck 4 to 6 weeks later. Men with risk factors for elevated hematocrit (sleep apnea, smoking, COPD, or a history of clotting) should check more frequently: every 3 months until the trend is stable.

* The monitoring schedule, the intervention ladder, and the attention...
