On TRT for 18 month Blood Work returned

Hi

I've been on TRT for 18 months. I have received my blood work back from private lab in UK;

E2 296 pmol/L (0-150)
SHBG 29 nmol/L (17-66)
Test 36.6 nmol/L (8.4 - 28.7)
Free T 2.63

Please advise on best way to get E2 under control.

What are the details of your TRT protocol? Injecting how much, how often? Remind us, please, of your age and weight? Managing estradiol in the UK is a challenge as you're not able to access the appropriate lab test, but there's no doubt your levels are elevated. Adding Anastrozole is one possibility, but so is adjusting your dosing schedule. More information may give us more context to respond with better suggestions.

I inject 250mg Sustanon over three injections per week. My age is 39, height 6'5" and weight 127kg. Please let me know if you need more information.

consider lowering your testosterone dose since Test is high 36.6 nmol/L (8.4 - 28.7) more is probably getting converted to estrogen
 
Adjusting your testosterone protocol, reducing your weekly amiunt should see your estradiol lower. The most important question, though,misnhow you feel. So...how would you describe your response to TRT?
 
Initially I felt great, really great, especially after years of suffering low T symptoms. Then about 6-8 months ago I started the slow decline back to feeling crap again.

Currently my libido has crashed, my belly is bloated and getting bigger, I'm getting fatigued easily and intermittent brain fog.

I guess that's down to the high estrogen.

If I lower my T, what number am I trying to dial in?

If decide to take arimidex instead of lowering my T how much should I take?

Would advise to test your Hemoglobin and Hematocrit with T levels elevated and high E2. Low dose anastrozole should bring your E2 back in line.

I've just booked an appointment with the Doctor for the above tests you suggested.

What kind of dose of arimidex do you suggest ... 1/2mg three times a week with my jabs? And how do I know if my E2 drops too low because of the AI?

Since you are unable to test via the sensitive lab procedure, you will have to rely on a close reading of how you feel with the test you do have to roughly guide you. Anastrozole is a potent drug - walk carefully.

Your doctor hasn't discussed anastrozole with you? It is remarkably potent: estradiol levels can be lowered to a point that a patient will feel just as, if not more, miserable than he did with elevated E2 levels. It can become a standard part of a protocol, many members here use it on a regular basis. Dosing is something that needs to be monitored. The "start low and go slow" mantra contains a lot f sense.
