New lab results need help!!!

Previously I had been on 100mg E5D and wasn’t feeling the greatest. Had lab work done and it showed the following:

TT 967 ng/dL
FT 277.7 pg/mL
SHBG 9 nmol/L
E2 51 pg/mL

Talked with the doctor and decided that since my SHBG was so low we would try 6 weeks of ED injections of 20mg.

I just got my follow up lab work done and it showed the following.

TT 865 ng/dL
FT 323.3 pg/mL
SHBG 9 nmol/L
E2 74 pg/mL

I do feel much better on ED and some of the symptoms I was experiencing have demished such as erectile issues. But I was shocked to see my TT wasn’t higher and that my FT and E2 had rose so much.

Any suggestions?

At these levels of FT out of range estradiol levels shouldn’t be a surprise. If you wanna lower these levels lower dosage or/and add an AI.

So you have NO complaints...no high E symptoms to complain about with that second set of numbers? With that SHBG you have a ton of Free Estradiol (something that should be run on Low SHBG guys) on the loose and I'd be shocked if you're not suffering through high E problems. We know that low SHBG = very high Free T = very high Free E.

No night sweats? Overly warm in bed? Angry/aggressive/emotional/PMS type issues? No nipple issues?

I’m feeling great. Any reason to be concerned about having high FT even though I’m feeling good?

Check cholesterol, BP, hematocrit, PSA, iron, ferritin, etc just to make sure that the high FT/E2 levels are not messing with anything . if all good I dont see a problem. Me personally don’t like the idea of being way off range when it comes to hormones (you never know what that may cause long term) but that’s just me

I will get my Free E results on Tuesday at my appointment, for some reason they do not come through on the app. Previously Free E has been very high.

I’ve not had any issue with bloating or any other symptoms of high E since changing to ED injections. Changing to ED has helped dramatically vs E5D. Could there still be room for improvement? Possibly, but I feel better then I have in a long time.

I’m typically hot natured but haven’t had any night sweats or nipple issues.

Unfortunately my doctor isn’t super educated when it comes to these things but is very open minded to allowing me to try whatever. This forum has helped me tremendously to get to where I am now.

I have an appointment on Tuesday. Was just concerned with the FT being to high and causing issues downstream.

I'd be more concerned with that E2 number, but if you feel good, no labs suggesting that E2 level is problematic, and no crying fits when seeing babies, then if it aint broke...What's your BMI or FB%?

I agree if you're asymptomatic...no worries. The thing you MIGHT have to watch with the Free T being so high is Hematocrit and/or Hemoglobin, along with the Estrogen. And PSA and DHT.
Know that being low SHBG you're just pissing out all that T but in light of not having any complaints, carry on, but your not having any complaints with these numbers is a little bid odd but I hope you're that "unicorn" kind of guy that makes it work, just contrary to what we know on low SHBG.

I’m trying to stay off AI if at all possible. Is there any reason to make a dose change?

your are the guy who caught the right protocol for you dont change a thing.. i was same guy as you had higher e2 and felt awesome just best time in my life until somebody adviced me to bring that e2 down and from that moment i never feel as good as i felt before lowering e2.. stay on this protocol and avoid AI

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
