Hi guys,

What's up :) 30 year old male from the Netherlands here.

I just got my blood work back from a lab today. I drew blood yesterday. I did the blood test because I thought I might have low T.

In short: I am not very energetic. Lazy. Low motivation (this bothers me the most). Not very horny, though I did have some morning wood the last week. But usually I don't. Weak boners with a girl sometimes. Not feeling like I am living my true potential.

Almost no (visible) muscle, I am not strong (I do not lift at the moment). Little bit of belly fat (not the kind of body fat you sometimes see in the US hehe, sorry). I eat pretty healthy but I am not to strict and also eat unhealthy things (fast food).

So here are my results:

Albumine
38.7 g/l
Ref: 35 - 55

Estradiol
0.02 nmol/l
Ref: 0 - 0.2
If I am correct 0.02 nmol/l = 5.44 pg/ml

LH
4.9 U/l
Ref: 0 - 10

FSH
2,3 U/l
Ref: 0 - 10

Testosteron
25.7 nmol/l = 740 ng/dl !
Ref: 10 - 35 nmol/l

SHBG
46 nmol/l
Ref: 20 -55

Free Androgen Index
56.1
Ref: 30 - 150

So my test is pretty high. Wtf! And my estradiol is really low.
I had my testosterone checked half a year ago (as part of some other test, I asked the doctor to test it too) and then it came out 17.4 nmol (=500 ng/dl)

I don't understand why it is so high now!

I read into this for about half a year on this site and also on other sites like dangerandplay. I really thought this is the problem I have since I have a lot of the symptoms of low T.

What do you think guys? Help me figure this test out.
What should I do?

Thanks in advance. I am very curious and interested in your opinions.

Cheers from rainy Holland!

The testosterone serum doesn't appear to be all that high. 740ng/dl is a good zone to be, and with a 4.9 LH value, it reflects you have some healthy leydigs.

I suggest you look more inward towards the thyroid. If possible, run Free T4, Free T3, Reverse T3, TSH, and antibodies (TPO & TgAb). Look at your iron and ferritin as well. The adrenals will be another area to concentrate on (cortisol & DHEA), especially if the thyroid has some negative results.

Thanks Chris! You mean that, judging by my FAI, my free testosterone isn't that high? Since it is, according to the ref values, on the low side. Does that mean that I am not getting the full benefits of the (normal) testorerone level? And how can I overcome that?

I will look more into the thyriod testing you suggested. I cannot get my doctor to do those test. So I will have to do it at a private lab and pay some money. I can get the things you mention tested but it will come at a price . I will do it ofcourse, it is my health and my life we are talking about. But I need to be sure this is the thing to do.

I have to tell you guys,

while ordering the blood work I (maybe prematurely) bought 2500 mg (10 x 250 mg) of Testosterone Ethanate online. It is of good quality and reliable. 100%. Being in Holland some things might be technically be illegal (in this case; without doctor prescription) but very easy and safe to buy and without any repercussions.

Do you think it is a good idea to use the T I bought? To get my T up to 1000 or something? And get my FAI better. I am not content with ''normal''.

What should I do, suggestions or any other opinions?

Cheers!

IMO, you would be better served to focus on reducing your SHBG a bit. This will effect your "free & bio available" test, which is essentially the FAI that you are referring to. The androgen index is your Total Test Serum / SHBG = FAI. Getting your SHBG into the mid 20's to low 30's will open up more free test.

Many people with higher SHBG can tend to be Vitamin D deficient, and E2 can have an impact. I can only refer to the E2 sensitive assay platform to gauge where a person sits. I have no idea where that assay you submitted fits in?

Anywho, again, Vitamin D can help (have yours tested), along with Netlle Root, Tongkat Ali, and/or some various anabolic compounds can work, BUT you really need to review it with a qualified physician.

To conclude for now, NO, I don't agree with implementing any testosterone enanth or cyp, or any exogenous compound. Your LH is decent, and your endogenous total serum production is excellent. If you could improve the free test % by a 1/2 a point or better, you would be miles ahead.

If you take the testosterone enanthate, your natural test (which is in good shape) is going to shutdown. When you are through with that bottle, your natural test won't be in the 700's anymore. After about 2 to 3 weeks, you will see it bottomed out. Your LH will also be low. You will then either need to look at a PCT, or you will need to continue taking testosterone the rest of your life. You don't need to mess with a good thing.

Focus on your free test & SHBG for that area, and focus on the thyroid and the other areas I mentioned for the energy /lethargic subject. Deal? TRT means replacement of testosterone .. You have testosterone, it doesn't need replaced ...

Deal! Thank you very much Chris.

I am curious how I would feel at higher T but of course I don't want to mess up somethings thats working good (according to the test report).

I will get the things you said tested and some other stuff (I will post the things I want to get tested on here, first do some more research). And I will report back.

Have a nice day!

You mentioned that you dont lift. Start to lift and you could probably raise your testosterone naturally if you want to know what it feels like. You'll easily put on some muscle too.

I took me some time but I am finally going to an Endocrinologist today. First I had to convince my doctor to do some test (he refused to test the thyriod stuff). The test came back and I had low T (13,2 ref range: 10 - 35) and my vitamin B12, D was also low. Unfortunately, like I said, my doctor didn't want to test for a lot of stuff you mentioned and things I read on this site & the internet.

I did alter the blood work document and included some vitamines (like the b12 and D) and estrodial. It's very easy to do. He gives you a blood work paper and you can cross things yourself. When he got it back he said: i dont remeber crossing estrodial. That is crazy you are not a woman. Why should that be tested. Which shows how much he knows :)

So I asked him to send me to an endocrinologist, which he agreed to after getting back the results. The waiting list was 6 weeks (yay, ''free'' healthcare) but today I will go. Very curious! I hope the endocrinologist is more willing to test for more stuff. I will report back the results.
