4. Print the prescription order form if you have ordered a prescription item.
5. For prescription orders, please provide the prescription with the following options:
   • Ask your doctor to fax us the prescription (604-581-4187).
   • Or, please email [email protected] or fax (604-581-4187) us a copy of your prescription, and we shall contact your doctor’s office to verify the prescription.
   • Or, please provide us with a copy of your pharmacy label with refills and we shall contact your current pharmacy provider to verify and transfer the prescription.
   • Or, mail us your original prescription.
