Brain Fog ,low concentration , Intrusive thoughts

Hello , I am new on this forum and hope I am not making mistakes in my exposure .
I am 38 years old ,173 cm x 85 kg ,ex atlete ,have always had good health and have never had surgery .
For about 4 years I have had an injury on my cervical spine which led me to several visits of various doctors with good outcomes in the end .
For about a year I have suffered from a certain mental fog that accompanies me during the day , peaking at night before falling asleep and upon waking .
Thoughts are swirling and I struggle to connect.
This sense of poor concentration accompanies me even during the day .
I evaluated my thyroid and the last two tests were as follows :
03-09-2024
TSH 2.65
FT3 2.49* ( limite basso)
FT4 1.14
While on 05-11-2024
TSH 4.22
FT4 1.32
Missing ft3
At this point I thought it was the thyroid but should I look into it?
I thought about diet , I have about 300 grams of carbohydrates on a 2,000 calorie diet , I do bodybuilding 4 a week .
I thought about low sugar (hypoglycemia)I have bad digestion (now the last 3 days i take enzyme digestivi )
My iron level is good and I take 20 mg of iron bisglicinate x3 time to week
Last visit to Neurology (this week) it everithing ok .
Does anyone have any other advice ?

You didn’t really mention anything about your diet other than carbs. Others on here have heard my talk about the benefits of the AIP diet, and for me personally when I followed it strictly for a month that’s probably the sharpest I’ve ever been. It also dramatically improved my digestion (which I often had issues with in the past). Digestion issues and the other symptoms you’re experiencing can almost certainly be improved to at least some degree by cleaning up your diet. Going full AIP for at least a month would probably result in the most improvement. I think the vast VAST majority of people these days are suffering from too much inflammation, and the AIP diet can certainly help on that front.

I follow an already very clean diet , use no spices and stay away from salicylates.
My carbohydrates are mainly from rice ,cream of rice in the after workout , bananas and occasionally oranges from my garden .
Protein is from animal sources so chicken , turkey , lean ground beef and occasionally eggs .
Fats are from homemade organic extra virgin oil,avocado in small amounts and walnuts or Brazil nuts (for selenium ),and rarely a piece of 100% dark chocolate.

What do you mean by “bad digestion”?
 
Have you been checked for sleep apnea? What is your height and weight? Have you had your thyroid antibodies checked? Testosterone?

For sleep apnea I have not been checked but I have a fairly regular weight (173 cm x85 kg ) , I have never been told that I snore or that I have apnea during sleep .
My thyroid antibodies I did not have it checked because by ultrasound my thyroid is normal and the doctor said there was no need ,but next days I will do the tests again .
Testosterone instead last analysis I had at 4.5, .

If T is 4.5 momo/L, that is your main problem. It’s super low. Get on TRT as soon as possible.
In your opinion are thyroid values to be considered normal ?
Doing the conversion t3 and t4 seem very low .
Before starting trt I wanted to use Dhea , what do you think ?
In my country to have a trt is very difficult believe me .
