My dosing schedule is 50mg of Testosterone Cypionate 2x a week. (100mg a week).

I am panhypopituitarism, that's why my TSH is low. I don't know what to do with my Estradiol. Should I go on an AI?

This is a tricky issue. If you feel great then you will get lots of opinions to stay the course. If you have symptoms you should give an AI a chance. I personally would not be comfortable with that E2 level regardless of how I felt.

You have room to lower a bit your FT if you wish but i doubt it will reduce you estradiol enough. Personally i would never let my estradiol that high even in the absence of symptoms. One thing you might try (specially given your SHBG) is daily injections. That will help lower estradiol but not sure it will fall within range but worth a try

I always feel hot, not sure if its from the raised estradiol though.

I'm always curious how someone feels before changing their protocol. How are you feeling?

I feel better with this protocol compared to once a week injections for sure. Feels smoother. My mood is more consistent.

Feel like anything is still lacking? Libido? Erections/E quality? brain fog?
You really haven't said that anything is still "wrong", for you.

Biggest things I can think of are achy joints, some hair loss, feel oily and feeling hot all the time practically. Libido is okay, erections are better with the trt. Brain fog has improved with twice a week injections.

If i were you i would try a low dose ai.

For the hair i would look into lipogaine or dualgen5 at night and essegen6plus in mornings...also add a ketoconazole shampoo eod.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
