5 Weeks on TRT-- Concerned My Numbers are Too High!

Hello,

I just got my latest lab results from Quest Diagnostics. Some of the numbers are still coming back but here is where things are at:

Total T: 1250 ng/dL (250-1100 ng/dL)
Free Testosterone: 406 (35-155 pg/mL)
Estradiol: 36 (<= 39 pg/mL)
Vit D 37 (30-100 ng/mL)

I'm feeling slightly better but still feel kinda depressed and having other hypogonadal symptoms like body fat, low energy, no morning wood, fatigue, depression, etc. I'm currently taking 200 mg of test cyp per week in two doses of 100 mg administered twice a week. Arimedix is 0.5 g a week. Should I cut back the test cyp to 100 mg per week divided evenly into two doses? Should the arimedix dose be increased? Thanks for your help!

Levels prior to treatment:
Total T: 400 ng/dL
Free T: 134.5 pg/mL
Estradiol: 32 pg/mL
Vit D 30

Your Testosterone serum levels are supraphysiological and probably why you feel like you do.

200 mg of testosterone injection is at the highest end of dosage for TRT; why are you injecting so much?

Case in point, most men who are age related androgen deficient and start TRT usually are in the 100 mg per week dosage range...you're twice that!

You need to reduce your dosage amounts.

E2 looks good but when you reduce your dosage you will need to reduce your AI dosage as well.

In TRT the line I like the best is "start low and go slow" and that is just good case management.

With such high dosage your SHBG levels are probably being suppressed as well; in fact, overall homeostasis is probably out of whack.

Reduce and get back to normal healthy levels.
 
My doc also likes to see Vitamin D in the upper range or at least 60-80. He feels that is the optimum area for well being and health. Do you supplement Vitamin D?

Gene, thank you for responding. 200 mg is what Dr Saya decided to start me on. Didn't you mention a while back that is the dose which the best trt doctors use with their patients?

In any case what do you recommend reducing the dose to? I also think I'm going to skip my next injection and then resume at a lower dose.

Defy tests their patients 90 days after treatment begins. I'm glad I decided to do it earlier.

Yes, I have been taking 6000 iu of vitamin d daily for the past 6 months.

My levels went from 28 to 37. Should I bump it up to 10000 iu daily?

My GP wanted me to bump up to 10000 iu w/ the exact same numbers. As Gene said you are supra. I would be surprised if Dr. Saya keeps you at 200 given your symptoms/side effects. FWIW I had very similar side effects when my TT and FT were elevated.
 
Make sure you are taking your vitamin D with fats. If you are like many people who take stuff at bedtime or in the morning on an empty stomach then you may not absorb it well. Do that before bumping your dose.
