29yo - need help breaking down results

29yo with lots of low T symptoms (low mood, ED, no morning wood, anxiety, no libido) but doc states these results are ok. It's my third test and all around the same. What does low FSH mean?

Prolactin: 312 mU/L (range: 86-324)
FSH: 1.9 U/L (range: 1.5-12.4)
LH: 7.0 U/L (range: 1.7-8.6)
Testosterone: 450 ng/dl (300-1000)
SHBG: 43 nmol/L (range: 16-55)
Albumin: 48 g/L (35-50)
TSH: 2.32 mU/L (range: 0.27-4.20)
Oestradiol: 55.00 pmol/L (range: 70-192 pmol/L)

I calculated my free test via SHBG/Albumin/TT and it came back as 1.6% (range is 1.5-3).

Any help/guidance is appreciated.

FSH stimulates sperm production. Your SHBG is part of the problem, it is binding a lot of your androgens and pituitary is increasing LH to attempt to compensate and increase testosterone, it's not enough. TRT is needed to decrease SHBG and increase free testosterone.

Most doctors are not educated in sex hormones, so your doctor doesn't know how to interpret your labs, best to go private and locate a doctor that specializes in TRT.

There is no specialist in managed healthcare that specializes in TRT, so most are operating with little to no knowledge and only fixate on total testosterone which is in range, but matters not because the free testosterone percentage is low which can be verified by directly testing free testosterone.


There is no chance of fixing this problem naturally.

TT 450 ng/dL, SHBG 43 nmol/L and albumin 4.8 g/dL would give you a FT level of 12.44 ng/dL reference range (16-31 ng/dL)

As you can see your FT levels are low due to your sub-par TT/highish SHBG levels.

Need to look into thyroid deeper.

Definitely look into finding a new doctor as yours is clueless.....low-T symptoms/low FT!

Thanks man. When i check ref ranges for free t i get 5-12 ng/dl?

For thyroid they look ok—
T4 free: 15.29 (range: 12-22 pmol/L)
TSH: 2.32 (range: 0.27-4.20 mU/L)

reference range of 164-314 pg/ml converts to 16-31 ng/dL
