Test P improved TRT, libido still at 0 – what next?

I'm a pfs case (but have the same exact issue as you, zero libido and also some brainfog). I also tried enclomiphene monotherapy but didn't help.

Really think it's something epigenetically. Btw many other things/syndromes with the exact same symptoms as PFS. I've seen people get "pfs" from Lion's mane, high dose zinc, AI's, saw palmetto, accutane, SSRI's, benzos like xanax, etc.
 
In my experience, enclomiphene is terrible for libido. It's very effective at increasing LH and testosterone, but the only benefits I got from it were a happier mood and more energy. Libdo was worse than when my testosterone was right at the bottom of the reference range.
Looking at the Enclomiphene subreddit it seems like it's very much hit or miss. It works for some and doesn't for other, including with libido. While it's clear that it's not the greatest solution to low T, I think it's worth at least giving it a try, if you're a younger person and have secondary hypogonadism. Of course I don't have any expectations that it would fix my libido.

Yeah who knows what's going on, but you're right that many substances trigger the same response as with PFS, so I'd imagine that the cause is similar, if not the same. The thing is that when I lost my libido I've never used any supplements or medication. It's weird to me how the symptoms that I had for 25 years were cleared 3 days into having hormonal fluctuation and proper DHT levels, yet libido still eludes me. I don't know man, our bodies are weird, I hope one day we know all factors in libido and can test them so that we can see where things are going wrong.

personally zero effect on Enclomiphene. Even the blood work didn't show much difference. Switched over to TRT, started little (800), no improvement. Then upped to 1000-Def libido boost, but died down towards end of that 3mos mark which was n is my TRT pallets intervals. Been on it since then.

For me, nothing works better than hCG for sex drive.
Do you find that sex drive improves further more on over 1000iu a week of HCG?

Do me personally I couldn’t imagine taking 2500 per week. I started out with 500 twice per week but have since adjusted down to 250 three times per week. That seems to be a sweet spot for me and I kind of just go by how I feel. Sometimes I might skip a day, sometimes I might take 300 ius for that day, but generally it’s the consistent 250.

I never really had issues with sex drive though, just reduced sensation and definite decrease in orgasm intensity. 500 ius twice per week completely fixed that within a month, but I started to wonder if my dose may have been a little too high due to constant low mood on the same day of the week (Sunday) and I tried to rule out other variables. Lowering dose and increasing frequency seems to have resolved that without a loss of any of the benefits. No way to know since I’ve never tried it, but I’d imagine if I did 2,000+ ius every week I’d probably be an emotional mess.
