Which type of treatment for Low T should I try next

So far, nothing has really worked with my low t except clomiphene. I have been on injections from my primary doctor, self injections from a men's clinic, gel from a men's clinic, inserted pellets, prescribed patch and androgel for under my arm.

Nothing has raised my testosterone as much as clomiphene, but it only raised it into the low 300 range.

I have had low t for at least 6 years, but probably had it longer but was not diagnosed.

My symptoms are basically.... Little to no energy, no muscle strength, I get winded walking up stairs, tired all the time. The list goes on....

i have been on this round of clomiphene for about two months and I can tell it is not increasing my testosterone.


I am trying to decide what to try next..... I am getting my blood tested again tomorrow, but I can tell it will be low.

With th all the options here, it seems that I have a few choices. Stick with my doctor and hope it gets better, go with Defy Clinic (instead of lowt.com), or use one of the suggested doctors from the list here on this site who just so happens to do pellets.


With Any and all options on the table, what should I go with? I just am not sure which to try next/again..

I am hoping to get a suggestion on what to do..

Welcome to Excelmale. Your struggles with TRT are astonishing; you simply can't raise levels under any circumstances to date? It isn't a question of posting solid labs, but not enjoying enhanced response? Is that right? Can you post lab work? If possible lab work done both before you began any sort of therapy, and lab work that was ordered during each of the therapeutic efforts you made. What is the rest of your medical history like? What other medications are you taking? How old are you? The more you can provide the better the advice you receive will be.
 
The bad thing is that I do not have that much data. As I switched to different clinics, i rarely saved the paperwork.

I do have three testosterone tests as my primary care physician and endo work for the same hospital system.

398- august 24, 2014
157- June 13, 2014

i take imitrex, protonix (just took myself off of this), pristiiq (for depression, but no depression medicine has ever helped me feel better, so I think it is something else), verapamil (migraine prevention), Botox (migraine prevention). I also take omega 3, multivitamin, fiber, etc...

Most people think think that I work out as I am 6' about 200 pounds, so I look healthy and am at what most think is a healthy weight. I am on a diet trying to get down to 190, but am also doing it because the wife wanted me to do it with her. I obviously do not feel healthy.

I definitely want want to help myself, but to be honest, I was wondering with all of the choices, which one should i do?

I cannot imaging that twice weekly injections of a decent amount of T would not get you to a good level where you feel good. If past doctors only got you to the 300 range, I am guessing that TRT was not their specialty.

Also, do you know your SHBG level by chance? Guys with low SHBG often don't really "feel" much from TRT, even when they look great on paper with a high total T, high free T and good E2 numbers - I know, because I am one of those guys.

What were your past protocols? How much were you injecting and how often? How many pellets were implanted and how frequently? When you used topicals, which ones, and what strength and volume? How long did you maintain each of these protocols and when did you test levels? This, as ERO noted, seems very odd.
 
I believe your testosterone levels are too low, you need to find a good doctor. Personally for me, I would only inject it's so much simpler.
 
Sorry for the late reply. Things have been crazy lately.

I had two sets of pellets inserted and it was a total of 6 each time. The first kind had trouble dissolving (or whatever they do), and I did not feel much different with either set. A couple of pellets had to be removed from the first set as they would not dissolve. I got a notice the other day saying the part of the company that handled mens health was shutting down.

The injections were from a prescription from my primary physician then from a men's clinic (same one that inserted the pellets). I do not remember how much I was getting, but my doctor gave the shot monthly and the mens clinic had me do it weekly.

The axiom (sp), was 3 swipes in my arm pit each day.

My my goal for this post was to see what everyone here things I should do going forward. I liked getting the pellets the best, but they clearly did not work.

The only thing that seems to do anything is the clomiphene. Can I take it and get testosterone at the same time?

Frankly, you were not well-served by your previous medical providers. As a number of us have noted, it is inconceivable that a well-managed TRT protocol will fail to raise your level of serum testosterone. Some men do not, subjectively, feel better, but even they obtain lab values that - objectively - reflect a response to exogenous testosterone. The key is "well-managed." A doctor only injecting you monthly is a fool. Weekly, or twice weekly, shots have been demonstrated to produce results. Anything less frequent have been shown to fail. Your first step is finding a doctor who understands this.
