Hi, guys. So i have a quick question. I've been doing intramuscular injections in my quad for some time now, just been wondering about one thing. I always inject in the area seen on the photo below. But its always somewhere around that area. Not in one exact spot. So my question to you guys is should i worry about hitting something else than a muscle? Or will i always hit it? I am using 23G and the whole needle goes inside. Thanks.

You should be good in that area. Avoid any surface veins if you can see them. There are no major nerves or arteries in that area if that’s what you’re asking.

And hell, get some 27g needles

I’ve been doing EOD injections in my quads since June 2017. I aspirate every single time, and have never had an issue. I’ve never aspirated blood into the syringe by the way, not even once. I use the outter quad, like in the picture, most of the time. Sometimes I inject closer to the top of my quad. Basically anywhere on my quad where the skin and subcutaneous layers are very thin.

And ya get a smaller gauge! You poor guy lol. I use a 27 gauge, just like fifty mentioned. Been using 27 gauge the whole time. I bet I could use a higher gauge if I wanted, but I just keep getting the 27 gauge needles out of habit

„23G and the whole needle“ - 23G is only a measure for the diameter of the needle, it doesn‘t say anything about the length. For the quad, I‘m using 23mm 25G needles (probably could go shorter).

I started with 27G and went a bit „thicker“ with 25G as I found pressing the plunger was a bit „shaky“ and I felt more comfortable.

23G definitely is somethign I would not do to myself.

Make sure to get quality needles - BD, or Terumi - as not all needles are alike

Thank you, guys. Yeah, i am kind of paranoid so i am using bigger needle to absolutly make sure that i am injecting into a muscle. I've heard some stories that subcutaneous injection can feel different for some people than IM injection so i didn't wanna take a risk, since i am feeling real good. So, like i said before, i am injecting just about anywhere in that black square, so pretty big area, hence that's why i wanted to make sure everything is ok. One more question. Should i worry about injecting into IT band? Or will i always go through it into a muscle with this needle?

Unless you are injecting more than 1 cc, I would avoid quads. Too many bleeding or post injection pain risks.

I've heard that shoulders are a good idea. Mark said once that changing my spot would be a good idea, just couldn't bring myself to do it yet.

Same for me. I've been injecting Test into my vastus lateralis EOD (alternating legs) for almost 3 years now, with no problems whatsoever. I'm still using a 29ga, 1/2" insulin pin to do this, and my bioavailability is just fine according to my blood tests.

I don't have any fat on my quads, so the 1/2" needle is going shallow IM, in my opinion.

If the OP, or anyone else wants to continue to harpoon themselves with a 23ga needle (and I'm assuming 5/8" or 1" length), have at it. I just don't see the point of poking big holes into your quad, when a smaller gauge and shorter length needle will suffice.

Side of the quad you will probably go through the IT band but that shouldn’t be a big deal. You could hit the same in the ventrogluteal area.

Injecting with a smaller gauge needle takes longer but it seems to leave me with less soreness.

I've done quite a bit of experimenting and perhaps what I say will sound careless to some, but all this fretting about subQ or IM, injection timing, location, and so on seems to me a big distraction. If you have valid reasons to avoid one or the other, then do what works for you. I find that my labs don't change no matter whether I inject IM or subQ, and I have no bad reactions one way or the other.

I started out worrying about all these nuances too, using 22 ga 1-1/2" needles. Now I use Nelson's method to combine T and HCG in the same syringe and I pin both of them together 3x/week. No AI. If I miss a morning injection because I'm traveling or something I don't fret about it - I just do it later in the day. No big deal. A healthy young male body does not have a constant hormonal state anyway.

A 27 ga 1/2" insulin syringe works for me in quads or shoulders or occasionally in the belly. I just pin it and go. ;)

What my wonderful endo gave me (3cc, 21g 1.5”) vs what I’m using now (1cc, 25g 0.8”)

I haven't found syringes with less than 1cc that do NOT have a fixed needle attached (would rather use .5cc or even .3cc syringes for more precise measuring), and I also haven't found .5 or .3 insulin needles that have a needle that's longer than 1/2" (.3 only 1/4") - I want to go IM and with 1/2" I wouldn't be confident I'm going deep enough....

If anyone knows a brand with either <1cc syringes or >1/2" long insuiln needles, let me know :)

(another, very practical reason why I'm asking for >1" long insulin needles: Over here in Europe I only get glass ampoules, not rubber-cap viles, so to get to the bottom of the ampoule I need a needle that's roughly 2cm / 6/8 inch long - and with insulin syringes, I coulnd't use a longer draw needle. Yes, I could backfill, but then it starts to get too complicated imho...)

The only syringes that come less than 1cc are fixed insulin syringes (.5cc/.3cc) and why would a fixed insulin syringe whether 1cc/.5cc/.3cc have a needle that is longer than 1/2 inch..... IT WOULD NOT because that length of a needle would never be needed to inject insulin!

makes sense... was just hoping that... well, there's a need for everything, so usually there is someone, somewhere, producing the things that might be a bit unusual. Doesn't seem like it in this case :) Maybe I'll give 1/2" shallow IM a try. and just draw out the rest with a regular 1cc + longer needle...
