PFS: Post-finasteride syndrome

PFS since 2019. Healthy male, 40's, also had two cycles of accutane in 1990s with complete loss of libido during those cycles. I am on TRT 30 units every 3 days with T of around 1,000.

I started Clomid and ED meds during the finesteride use without connecting any dots at the time. Total loss of libido was ignored by Dr, and I also ended up with all the other side effects. My trial of PT-141 started 2 weeks ago, and here is what happened.

PT-141 10mg vial. Reconstituted with 1ml BS water. Starting dose was one unit in early am. I was hoping for a very conservative dose for safety sake, and to hopefully avoid nausea or priapism. No side effects from this dose and almost no immediate effect. I did get a slight bump in libido that was very temporary during this period.

I continued 1 unit dosing every few days in the morning, and over time started to get indications that some libido was returning. I increased dose to 1.5 units after these positive indications which resulted in a day with "normal" libido. I missed my ED med that day and had intercourse without needing it. The big surprise was a total restoration of semen quality from prior to PFS onset.

During the worse part of PFS I had watery semen that was very low volume as others have described. In partial recovery from PFS I started having less watery semen that was lumpy. During the 1.5 unit day I noticed strong cowper's gland excretions like I remembered from my youth. During intercourse I experienced a full creamy load of semen of normal appearance/volume. Something I haven't seen in years!

I have had very positive results from other peptides as well, which started me down this rabbit trail of peptide research. I hope this will help someone suffering like me. Let's chat....

I also came into this with PFS and I’d been trying PT-141 off and on over the past couple of months. My sperm was like yours with PFS in that it became watery. HCG solved that for me though, and now it’s so thick it starts conversations. The higher my E2, the thicker my sperm seems to get. AI’s lowered that. Right now it’s thicker than it’d ever been in my life. I have issues with hydration, so it’ll tend to be more ***** the less hydrated I am, approaching Jello.

Most PT doses I’d read about were 2mg once a week or so. I tried 1mg a week. I’d notice a little nausea on the first day in the morning after taking it. Usually wouldn’t notice much until the 2nd or 3rd day, in which case I’d think about sex more and thoughts of it were more pleasant. Had no affect on erection or sperm quality for me, but I came at a lot of PFS issues through other avenues. No side effects I could tell from PT, other than the momentary nausea on day 1 in the morning. Thyroid hormones while on test and using things like adaptogens or cortisone acetate had more of an impact on that for me. Rhodiola and thyroid hormones on test had the biggest effect. Lowering my test also increased my penile sensation, but lowered my libido alongside it.

In the future I don’t know if I’d bother with PT again. I have another bottle I might try micro dosing daily with, but 10 units is already a really small dose to get 1mg for me, and I don’t think I could productively get 1-2 units of something into a syringe. I could add more BAC water but I don’t know if that’d fit in the bottle.

Prior to PFS and any of this stuff, I’d say I usually had low libido, and my sperm was relatively watery. I could have sex any time I wanted for as long as I wanted and my dick was great, but I didn’t often think about sex without being prompted by something, usually visual. Never really had morning wood or anything, basically ever, after I was a teen. Only time I’d ever gotten consistent morning wood was on test and messing with thyroid hormones and adaptogens while on 2.5mg of Cialis. I’m on Cialis now but my issue’s more getting my penis to retain blood, which for me is most strongly influenced by thyroid hormones.

Speaking of PT141, I haven’t used it for years, not since I started using Trimix, but I just decided and have ordered some PT 141 With the hope that it will enhance my ability to orgasm. On occasion i have some failire to complete the deed.

Anyone have any comment?

Curious how it works for you Bremster. I recommend only use a product from a reputable compounding pharmacy rather than an internet peptide site. If you can get Vyleesi, I would recommend trying that over PT-141.

Just to be clear, you're using U-100 syringes and your doses have been 100 and 150 mcg per day? I am intrigued. I have little doubt that milligram-sized doses are well into supraphysiological territory when it comes to activating the melanocortin receptors. I'd thought it would be interesting to run a trial with smaller doses to see if there would be any benefit. If you continue this treatment I hope you'll keep us updated on your results.

My experience with PT-141 is that the time for it to "kick in" is dependent on dose. At a 10 unit dose (1 mg of PT-141), it takes over 8 hours before I feel the effects. At a 15 unit dose (1.5 mg), 4 to 6 hours. At a 20 unit dose (2 mg), about 3 to 4 hours. If I take more than a 2 mg dose, I get severe nausea (and I mean howling up your socks nausea).

When PT-141 kicks in on me, I get a warning "chub" about an hour before it's party time.

However, I've also noticed something else. If I take a 1.5 mg dose on day one, it works well [for me]. I'll have spontaneous erections for the next 24 to 36 hours - erections that wake me up in the middle of the night, almost to the point of being obnoxious. Better half always asks "Again? It's 3 AM - gosh, aren't you done yet?"

Next day, I can take a 2 mg dose and get next to nothing out of it. I have to wait several days before noticing any effect from PT-141 after the initial dose. If I take PT-141 more than once per week, it's just emptying my wallet. Guess it burns out the receptors.

Never thought about daily micro-dosing though. Interesting topic!

Defy Medical is a good resource for prescriptions from Empower pharmacy. For Vyleesi, your wife or girlfriend can get an Rx from their MD, then share. There are special price reductions direct from manufacturer for women too.

Yes, U100 syringes but I'm using the insulin .3 ml size so the resolution of small doses is good. I definitely wanted to start with small doses and work up from there if needed. I can tell you the 100-150 mcg dose is noticeable with flushing within a few min.

This isn't a silver bullet by any means, but I'll take any positive results after the PFS started.

regarding about adding pt141 to my presexual routine, allow me to elaborat. I purchased some PT 141, but two days before my anticipate event I got Covid boaster, the latest, my fourth one. Based on what I could learn on line, I injected .2 ml PT 141 plus my usual Trimix injection.
Everything went off almost too good, I was first to reach my orgasm.
Then things starting going south. Headache, persistent erction, flue like feeling, then late in the afternoon I threw up four times until there was nothing left, that was yesterday, I still feel like hell. I was concerned about doing major damage with this persistent erection, but it begin to fade after several hour, not completely going away but by end of the day it was almost flaccid.

I don’t know how much was from the covid shot and how much pt141.

if I try pt 141 again I wI’ll want to scale back on the dosage, anyone have a suggestion?

Back up a little. How many milligrams of PT-141 in your vial originally and how many total milliliters of bacteriostatic water were added to it?
Your dose then was 0.2 mL * (milligrams PT-141 in vial) / (mL bac water added to vial)

PT-141 would not be my choice for something to help with orgasm. Have you measured prolactin? Difficulties with finishing can reflect an imbalance between dopamine and serotonin. Enhancing dopaminergic activity is sometimes helpful.

So you took 2 mg, which is indeed a high-end dose, and explains your side effects.

I've been wondering how to determine the relative effects of this peptide compared to endogenous hormones. A quick and lazy approximation notes that concentrations by weight of α-MSH are comparable to those of estradiol. If this equivalency is in the ballpark then standard doses of PT-141 could effectively result in levels at least 10-20 times higher than baseline.

I tried 0.4 mg about 2 hours before sexual activity along with my usual Trimix. Erection no problem, but no orgasm. As indicated in my previous post, 2 mg made me deathly I’ll, but orgasms was no problem.
with the light dosage I had on and off erections while trying to sleep 24 hours later. I think I’ll give up on pt141 as Anorgasm cure.
