Hello

I'm 39 years old.
Since 10 years, I have no libido, I don't feel desire, I have ED.
I had a long time with Low -T (450 ng/dl), but now is ok. (665 ng/dl) without any TRT.
But still I have no libido, and no any improvement in my sexuality.

Looking for reason, why it looks so bad.
Maybe Estradiol is to low ?

Testosterone - 665 ng/dl
Estradiol - 16-18 pg/ml
SHBG - 42 nmol/l

10 years ago I had testosterone - 700 ng/dl,
estradiol - 45 pg/ml,
SHBG - 21 nmol/l

I did't take any IA.
I don't have depression.
My other lab test are good.
I have morning wood, but no libido.

What should I take to boost Estradiol ?

Coming off a period in which I had undetectable estradiol, I can tell you that my main symptom was having no libido. The problem in your case is that you can't use exogenous estradiol without suppressing your testosterone. You might see if very low doses of Clomid are helpful. Clomid is somewhat estrogenic on its own and does stimulate estradiol production.

Estradiol level is 3 times smaller.
This is strange.

I believe Clomid will increase SHBG, which explains why it went up.

Also I think once you stop Clomid you lose all the benefits, so you need to stay on it (like you need to stay on TRT).
 
Just before I got clomid, SHGB was also about 41nmol/l, E2: 10-15 pg/ml.
So, it didn't change anything with SHBG level.
I didn't feel much better with clomid teraphy.
But it restart my pituitary very well.
I train in a gym to keep testosterone high level.
I don't wanna back to clomid.

I’m confused, are the numbers you posted before or after Clomid? In that 10 year gap your SHBG doubled, so it could have played a part in that.

Before clomid- SHGB - 41 nmol/l
E2 - 10-15 pg/ml
Testosterone - 450 ng/dl


10 years ago- SHBG - 21nmol/l
E2 - 45 pg/ml
Testosterone - 700 ng/dl

Now (after restart) I have good testosterone but low E2 16-18 and SHGB 42
 
It's common to see men with SHBG on the higher end with low Free T and lower E2. You increase estrogen by increasing testosterone, not likely to ever happen approaching 40 because your levels are on the decline and will get lower and lower as you age, SHBG higher and higher as you age binding up more and more T.

I'm willing to bet that Free T is suboptimal and your E2 is lower because Free T is not where it needs to be. You can't beat mother nature at this game unless you go on TRT.

Nobody does that if testosterone level is normal.

My free testosterone - 12.3 ng/dL = 1.89 % I think, it's not so bad result.
(I used calulator - Free & Bioavailable Testosterone calculator)

I had some sex with Viagra, but it didn't help too much.
Without Viagra I'm almost death, but this drug works better with sexual arousal.
When I have sex sometimes I feel arousal, but only few seconds, and if I'm on Viagra in this time, it helps me to get erection.
But still it's very poor sex, more like with 70-80 years old men, who is fighting to get any erection.

I have joint pain besause long time ago I had very bad arthritis inflammation.
I recovered, but doctor say that I will feel some pain to the end of my life, and I have to get used to.
So, it's difficulte to say that low E2 is guilty.

If you say so, but a normal Free T percentage is between 2-3%, your below normal. According to Dr. Abraham Morgantaller an expert on testosterone, states men at or below 15 pg/mL (6.8-26.5 pg/mL) are expected to show benefit on TRT.

Would you also say your erection quality and sexual satisfaction is not a bad result? You are overly focusing on the numbers and ignoring the symptoms that are plainly obvious.

Joint problems are common to those with suboptimal testosterone and estrogen.

Testosterone is responsible for nitric oxide production which the penis requires to function properly, without it the penis will become weak and not function properly. I bet if you tested nitric oxide, levels would be low.

Viagra can't work if free testosterone is low, ED drugs are for those with healthy youthful testosterone levels where the ED is caused by structural problems with the penis, not for those with a testosterone deficiency.

You've been given the wrong treatment in an age where most doctors aren't up to speed on male sex hormone knowledge and countless men are being told they're normal when in fact they're not.

By the way that calculator you linked, it's inaccurate. It's probably overestimating your Free T levels.

If you truly want to know where your FT levels sit than you would need to have it tested using the most accurate assays such as the gold standard Equilibrium Dialysis or Ultrafiltration (next best) or simply use the newer calculated TruT method (which has been shown to be on par with results obtained by the gold standard ED).....it is available online to the general public for free.....TruT Free Testosterone Calculator by FPT

If we take your TT 665 ng/dL, SHBG 42 nmol/L, Albumin 4.3 g/dL (mean) than your FT is 21.78 ng/dL ( just under the mean 23.5 (mid-range) of the reference range of 16-31 ng/dL)

As you can clearly see that although your TT levels are descent and your FT level is by no means low (just under mid range of the reference range).....it is definitely sub-par due to your highish SHBG.

Most men do well having FT levels in the 30 ng/dL range.
 
You can 100% get on TRT with those numbers if you want to. Obviously not through insurance. When I started my total was 691. But my free at was very low like yours, due to a high SHBG. Almost every good TRT doctor that doesn’t take insurance will give you the option to start therapy if that’s what you wanted to do.

The answer to your original question is testosterone. How to boost E2 is extremely easy. You just take testosterone. Couldn’t be any simpler. Just hop on TRT and start enjoying your life again.

But there is a problem with this.
When my TT level increase to more than 680-690 ng/dl, I feel anxiety, I have water retention, ance, and very very bad joint pain.
My body realy doesn't like to higher TT level.
And I would like to keep fertility (I don't have children yet), what is difficult on TRT.

What kind of TRT do you recommend ?
HCG + testosterone gel ?

PS. My FT calulator really sucks, thanks for the hint.
I will do lab test anyway.

What do you mean, you have been on TRT before? Injecting T infrequent can cause water retention, inject small frequent doses should solve the problem, that's one of the reasons why I inject daily.

Testosterone gel is ineffective for the majority of men, however the T-cream applied to the scrotum is better absorbed, but injections still is the most effective method. I use 29 gauge insulin syringes and inject daily in the shoulders and quads, I feel nothing otherwise no way would I inject daily.

When you want to have children you can add HCG and FSH injections to your TRT protocol, then hop off when the deed is done.

I used clomid 6 months ( finished 3 months ago) 12-25 mg E4D.
After this time my testosterone increased from 450 to 650 ng/dl.
Clomid gave me good restart, but didn't improve libido.

12 years ago, when my testosterone first time crashed to 300 ng/dl, I had 12 months HCG teraphy.
I finnaly got 700 ng/dl, but I didn't hold it for several years.
During last 12 years I went down to 450 ng/dl, but I don't now exactly when it was.

I didn't have therapy with testosterone.
HCG only or Clomid.

Man, your posts are always encouraging. Have you tried HCG only?

Since we're talking about shbg, my shbg is around 60, e2 is 19, and free e2 was well within range.

I love helping people, so if I encouraged even one guy on here, that makes me feel good. But yes, I tried HCG only for a few years when I first started HRT. On 2,000iu’s/ week, my total T went up to 1200, free T was upper quartile of the range, SHBG went down from 50’s to the 40’s, and E2 was around 50 or so most of the time, while using an ai. So numbers looked pretty good. Subjectively my sex drive went up, and my mood improved, if I remember correctly. The only reason I had to stop was because I couldn’t get rid of the itching/ sensitive nipples, and I started to get gyno at one point. The nipple issues were just a reoccurring thing. I never tested prolactin or progesterone while on HCG mono, but knowing what I know now, my guess is that the HCG was increasing one of those too much. I’d put my money on prolactin.
