What kind of T - therapy would you recommend based upon these numbers?

Missing from your labs are TSH, Free T3, Reverse T3 and antibodies, Free T3 is the most potent thyroid hormone that is responsible for body temperatures and metabolic rates. Low SHBG can be genetic or the result of low Free T3, diabetes (insulin resistance) and obesity.

A lot of doctors were taught to only check TSH and Free T4, times have changed and sadly doctors have not, Free T3, Reverse T3 and antibodies show a more complete picture of thyroid status. Free T3 shows what's going on at the cellular level, TSH and Free T4 does not.

If not hypothyroid, I would recommend frequent injections do to very low SHBG, at least an 20-25mg EOD protocol and if you have difficulty controlling estrogen, a 10-12mg daily protocol would be best.

Because of your low LH and FSH you have secondary hypogonadism. This type of hypogondismf indicates a problem in the pituitary gland that signal the testicles to produce testosterone. So I would recommend trying low dose clomid first. It may be enough to wake up your testicles and make you feel good again.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
