Defy Medical TRT clinic doctor
been having weird symptoms recently. got tons of tests and MRIs done and no answer. My bloodwork for Thyroid came back like this. Not sure what to make of it as Im not knowledgeable on Thyroid. My primary told me only to test TSH, but my HRT doctor said to get a full thyroid panel.

Anyone know what to make of this?

I have elevated thyroiglobin antibodies, lots of people have elevated antibodies and no symptoms. This may or may not be Hashimoto’s disease where the immune system attacks the thyroid gland.

Hashimoto’s disease doesn’t always require treatment.

I was gonna recommend him to optimize his diet, in order to get his antibodies down, but aren’t u basically doing carnivore? I’m surprised u still have elevated antibodies when ur diet is already so optimized

I usually associate autoimmune stuff with excess inflammation throughout the body. Do u think there’s a chance u have elevated levels of inflammation atm? Do u have any theories on what could be causing ur antibodies to be elevated?

  • Thyroid peroxidase antibodies (TPOAb). High levels of these antibodies are a sign that Hashimoto's disease is causing hypothyroidism.
  • Thyroglobulin antibodies (TgAb). High levels of these antibodies are a sign of Hashimoto's disease.
  • Thyrotropin receptor antibodies (TRAb). These antibodies can be a sign of Graves' disease.

It’s very interesting to show a low TSH with the thyroglobulin antibodies you have. Can you have the test run again? Is this the first time you have tested thyroid function?

Thyroid antibodies are proteins that form in the blood when the immune system mistakenly attacks thyroid cells and tissues. This can lead to inflammation, tissue damage, or disrupted thyroid function. High levels of thyroid peroxidase antibodies (TPO) indicate an ongoing attack on the thyroid, and may put you at a higher risk of developing a thyroid disorder like hypothyroidism in the future. However, having high thyroid antibodies doesn't necessarily mean you'll develop hypothyroidism. If you have high thyroid antibody levels but normal thyroid function tests, you probably don't need treatment with thyroid hormone. Instead, you can try lifestyle changes like optimizing your nutrition to help manage the disease.

Ive only run TSH in the past and it has always been in the 1.0-2.0 range. Maybe ill do another, but its kind of pricey to get the full panel, so probably down the line

Not pricey if you live in the US (34 states)

What symptoms made you check your thyroid? How are you total and free testosterone?

so for the past 4 months Ive had issues with intese nerve pain that ranges from pins and needles to sharp pain all over my body: hands feet, sometimes chest and even face, similar to neuropathy

I have had many many tests done MRI on brain and cervical region, Nerve testing (EMG test), cbc, CMP, heavy metal testing, Lyme, vitamins, xrays on spine. etc.. nothing showed up. everything looked fine

finally I talked to my HRT doctor who suggested a full thyroid panel as my primary only recommends TSH. So I got that done and these results came back. Also had my annual HRT physical done and the physician said my thyroid seemed slightly swollen

Man, I am so sorry to hear. It must be horrible and frustrating. Are you taking any medications?

Lupus, maybe? I would run the test again, though.

High thyroglobulin antibody (TgAb) levels in the presence of normal TSH, T3, and T4 levels can be indicative of several conditions, primarily related to autoimmune thyroid disorders. Here are the main causes:

1. Autoimmune Thyroid Disorders

  • Hashimoto's Thyroiditis: This is a common cause of elevated TgAb levels. Hashimoto's thyroiditis is an autoimmune disorder where the immune system attacks the thyroid gland, leading to chronic inflammation and often hypothyroidism over time. Even if thyroid hormone levels (TSH, T3, and T4) are currently normal, the presence of TgAb suggests ongoing autoimmune activity against the thyroid.
  • Graves' Disease: Another autoimmune disorder that can cause elevated TgAb levels is Graves' disease, which typically leads to hyperthyroidism. However, in the early stages or in treated individuals, thyroid hormone levels might be normal while TgAb levels remain elevated.

2. Thyroid Cancer

  • Thyroid Cancer Monitoring: Elevated TgAb levels can interfere with the measurement of thyroglobulin, which is used as a tumor marker in thyroid cancer patients. The presence of TgAb can indicate residual thyroid tissue or recurrence of thyroid cancer, even if thyroid hormone levels are normal.

3. Other Autoimmune Conditions

  • Other Autoimmune Diseases: Elevated TgAb levels can also be seen in individuals with other autoimmune conditions such as systemic lupus erythematosus, rheumatoid arthritis, type 1 diabetes, and celiac disease. These conditions can coexist with normal thyroid function tests.

4. Subclinical Thyroiditis

  • Subclinical Thyroiditis: This condition involves mild inflammation of the thyroid gland that does not yet affect thyroid hormone levels. It can be an early stage of autoimmune thyroid disease where TgAb levels are elevated, but TSH, T3, and T4 levels remain within normal ranges.

5. Benign Thyroid Nodules

  • Benign Thyroid Nodules: Elevated TgAb levels can also be associated with benign thyroid nodules. These nodules can cause localized inflammation and immune response, leading to elevated TgAb levels without altering overall thyroid hormone levels.

6. Iodine Imbalance

  • Iodine Deficiency or Excess: Iodine levels can influence thyroglobulin levels. An imbalance in iodine intake can lead to elevated TgAb levels as the thyroid gland responds to the altered iodine status, even if thyroid hormone levels remain normal.

Conclusion

Elevated thyroglobulin antibody levels in the presence of normal TSH, T3, and T4 levels are most commonly associated with autoimmune thyroid disorders such as Hashimoto's thyroiditis and Graves' disease. They can also indicate other autoimmune conditions, benign thyroid nodules, or be a marker for monitoring thyroid cancer. It is essential to follow up with a healthcare provider for further evaluation and management based on the specific clinical context and additional diagnostic tests.

I agree that he needs to find out why the antibodies are elevated, most likely it’s something he’s being exposed to or eating.

I am doing carnivore with the exception of having to drink 9 ounces of orange juice every morning. No more than 23 grams of sugar per day, and with my activity level, I don’t see that making much of an impact at all.

The carnivore diet is best for getting the lowest levels of inflammation! If he goes on a carnivore diet and his antibodies go down, he knows it’s something he’s eating and or something he’s lacking.

any idea why my values may be like this?
Antibodies I would associate with inflammation. So would just try to eat as clean as possible/ remove all inflammatory foods, or as many as u can, to get those down. My thyroid antibodies have always come back basically as low as the range goes, whenever I’ve tested them.

To get free T3 up, I would simply try to get more iodine in ur diet. So drink pure cranberry juice, consume pastured egg yolks, seafood, grass fed dairy, to mention some pretty good sources. If u can’t get enough via diet, to raise ur free T3, a little lugols iodine should do the trick. Just one drop per day of lugols 2% solution boosted my free T3 up a bunch

And to lower reverse t3, I would again, consume more iodine, as well as more selenium. I’ve heard selenium can be pretty helpful when trying to keep reverse T3 low. On top of those things, I would again just try to optimize ur diet, as much as possible, and remove as many inflammatory foods as possible

Just try to consume the foods I mentioned. And like I said, if u can’t get enough from food, to see a boost in free T3, u can simply just take 1 drop of lugols 2% iodine solution per day. I can link u the one I’ve used in the past. It’s super cheap
