So I met with an endo on Friday. It was awful!!! I originally tested for Low T in 2014 when I was 28. It came in at 199 total. My PCP put me on injections of 200 cyp once a month. It was great for the first 2 weeks but would seem to wear off. I got a total of 2 injections and finally started to do research on it. I went to a urologist who took me off TRT injections and told me to take take arimidex 1 time a week for 4 weeks. My number went up to 420ish and he said I was good to go. I told him I still feel awful and he said “well if you want babies you’re gonna have to deal with it.” So I switched urologist at the same office and have a urologist who is awesome but wouldn’t put me on TRT because of fertility. He put me on clomid 50mg ED and have been doing that since 2016. Well I got bad side effects with and the highest I tested was 520 on clomid. I did a test 3 weeks back and the numbers came out at 478 and retested at 520 again. I don’t feel any difference and still having fatigue, low libido, brain fog, and just don’t feel well. I lift weights and hurt for about a week at 30 years old. It’s awful.

So I went to the endo and she said I needed to have two low tests below 300 to even consider TRT. She said my testosterone levels are fine and that’s this is something I have to deal with. She wants to run tests to make sure there is nothing else going on. I was so mad because my co-pay is $70. Waste of money because I had blood work done in January and just had my yearly physical and everything was fine. I also asked her about my high SHGB which cam in at 65.5. She said it doesn’t matter. She wants me to go back in 6 months and said I’m perfectly fine. She also told me I’m too young to even be on TRT. I told her well I don’t feel fine. Anyway I’m not going to follow through with it but I’m really upset with this whole process. I have an appointment with a fertility urologist on Thursday and am basically losing hope. Any suggestions? Do I have to continue living exhausted and feeling like crap? I know I can go to defy or a low t clinic but those are so expensive that I can’t afford it. I’d like use hcg testosterone and clomid (when trying to get my wife pregnant.) I called around and target near me has testorone vials for 25$ without insurance. It’s just crazy how little that doctor knew. She also told me HCG has way too many complications to give me.
Also do you think maybe because she was a female doctor and really doesn’t understand what I’m going through? (Not meaning to come off sexist but am curious about that.)

Anyway here’s is my last labs:
(Note: I was taking Clomid 50mg daily for 3 months and stopped a week before the tests minus the albumin and TSH in January)

Testosterone Free: 520
SHBG: 65.5
Estraidol: 24.8
Hemoglobin: 17.0
PSA: 0.7
Albumin (in January): 4.4
TSH: 1.37
Any help would be appreciated!!!!

So what's your saying, your testosterone levels is 520 and you're on no trt protocol? You do have high shbg, which my cause symptoms.

The majority of your testosterone isn't available to your body and is why you feel unwell, most doctors are ignorant when it comes to male hormones and doctors refuse to give young men TRT for fear of being slapped with lawsuits for giving a young man TRT. This is the real reason they keep telling you to just deal with it.

I don't see any labs for thyroid.

No. Only thing I was given is clomid 50mg ED and haven’t noticed a difference in erections, libido, fatigue etc.... I only get side effects from the clomid. Sore joints, headaches, some vision issues. Nothing blinding but see zigzags some times. I just can’t he keep taking that and living like this. The joint soreness is so bad that when I do push ups my wrist hurt for a week or so. I tested this and quit taking it for about 2-3 months a year ago and no more wrist pain, shoulder pain or knee pain and ankle pain.

All I was given to take was the TSH ultra sensitive which came out at 1.37. I’m not sure what else I needn’t for thyroid test though.

TSH is a stimulating hormone that is excreted by the pituitary gland which stimulates the thyroid gland to produce thyroid hormones, except you don't have any actual thyroid hormone tests which indicates your doctor isn't very knowledgeable.

Typically checking Free T3, Free T4, Reverse T3 and antibodies will show a more complete picture of thyroid function.

Unfortunately you will soon realize that going to endo's or “regular” family doctor will not get you very far when it comes to hormones and levels that are not “optimal”.

Lab ranges are primarily to help the insurance companies from paying out for treatment. If you are “in range”, then no treatment.

Once you are tired of playing games with the main stream medical system, you will need to seek out clinics/doctors who specialize in TRT.

As mentioned already, with your high SHBG number, your free T is probably very low. This is probably the reason for your symptoms.

Good luck, and hope you decide to move onto a doctor/clinic that will help you.

Don't discount this part too much. I had all the symptoms you describe and there was other stuff wrong with me that is alot worse that low T that the Dr found in addition to low T. At your age, I would wonder why you are having symptoms. Could be low T as other suggest, but 520 by itself is not that low.
PS Slight edit, I see that 520 is not your natural state. Yes 199 is low. Not sure what your current non-supplementing numbers are right now, But I would still want to know why at your age you were at 199.

In range ends all discussions, this is the point where doctors tell you labs are normal even if you clearly are showing symptoms. I'm certain doctors would prefer to take action, but insurance companies are pulling their strings and makes the doctors look like cold, uncaring robots. It couldn't be further from the truth, it's the insurance companies who are cold and uncaring and makes paying cash for healthcare seem like the better, healthier long term option.

Even if I am bein treated for low t already? I really don’t needn’t the insurance company to pay for the prescription because the testosterone is cheap at my pharmacy. I pay a crap load of money for monthly supply of clomid which my insurance doesn’t cover.

You will get great feedback from folks around here.
1st order is some self educations to get a better understanding of TRT and HCG.
Download DR. Johns papers on TRT and HCG for a good basic info.
Some thyroid links for education to see if anything sounds familiar. Also discussion's on basic thyroid test, iron test, etc. that will give a better picture.

Cannot tell how much research you have done on TRT.
If not a lot, I recommend you get a few books.
Get a few good books and educate yourself now:
1. Get Nelsons book "Testosterone: A Man's Guide".
2. Get Lee Myer's book "Natural Versus Testosterone Therapy".
3. Get “Testosterone for life - Dr. Abraham Morgentaller “.
4. Get “The Definitive Testosterone Replacement Therapy MANual: How to OptimizeYour Testosterone For Lifelong Health And Happiness” by Jay Campbell.
These books will give you a much better understanding of TRT....
