I'm in my early 40s. The free testosterone with equilibrium dialysis method is not available in my country. Total testosterone is 402 ng/dL The vermeulen equation after putting the Free testosterone (by CLIA) 15.7 pg/ml and SHBG 32.7 nmol/l, shows the following results

Are we dealing with significant T deficiency? There are symptoms of hypogonadism. LH 3.92 IU/L and FSH 4.65 IU/L. free androgen index 42%.
 
Assuming this is morning testosterone: Your cFTV result of 8 ng/dL is borderline. Some men do have symptoms at this level, even though it is considered to be within the normal range.

I would be hesitant to start conventional TRT without more information. If you have access to short-acting testosterone such as testosterone nasal gel then a trial may confirm hypogonadism by resolving your symptoms. Enclomiphene is another possibility, though results are less predictable.
 
It depends on how you are sleeping, recreational drug use, body weight/fat, and other factors. Your T is borderline low but many doctors would not treat at that level, and your calculated free T percent is good.

I would call defy medical. They'll take care of you. That's who I go through.
