Hi,

I would love to get your thoughts on these labs. Age 38, M, 210lbs, 5'10".

Measurement

Range

TESTOSTERONE, TOTAL, LC/MS

232.1ng/dl

264.0-916.0

FREE TESTOSTERONE(DIRECT)

7.6 PG/ML

8.7-25.1

Estradiol, sensative

19.4pg/ml

8.0-35.0

SHBG

12.5nmol/l

16.5-55.9

The low SHBG kind of confuses me. Wouldn't having a low SHBG mean that I would have more free testosterone? Does having a low SHBG make me harder to treat?

Low SHBG means more free testosterone than if SHBG were higher. But the difference isn't quite as big as we used to believe. Your very low total testosterone still translates to low free testosterone. The Tru-T calculation says free T is about 8 ng/dL, which is abysmal when the normal healthy range is 16-31.

Low SHBG does make it harder to treat. The reasons why aren't totally clear, but may include having high free estradiol relative to free testosterone, and a deficiency of intracellular testosterone.

Your total T is low so Free T will only be so high. Raise your total T and free will go up.

I have low SHBG (10-13 off TRT/22 on TRT) and inject 7mg daily (49mg weekly) and feel great, however if I inject every other day and even every 3.5 days, I feel less benefits and don't feel as good. When you double your testosterone, I expect estrogen to at least double which would make you estrogen dominant.

Adjusting the frequency and size of your injections can push testosterone higher and estrogen lower.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
