Hey all I know I've been asking a lot of questions so I appreciate your patience cuz I'm going through a hard time. Over this weekend I've experienced insane rage followed by tears. My understanding is this is typically caused by high E2. My dosage is 240 mg a week and has produced a T level of 839 and an E2 of 77 ( which is prob a bit high based on him taking blood at peak and taking the nonsensitve test). If I get my doc to put me on .25 arimidex 2x weekly, how soon can I expect the mood changes to reverse? Days,weeks, a month? Also, how long after starting should I test my blood levels again? Thanks. Mike

There's a lot wrong there...you didn't use 240mg and pull a 839 at peak, using the wrong testing at the wrong time...you need a new Dr more than trying to add Anastrozole.

Well blood was taken about 18 hours after injection so not peak, but I'm taking .6ml of 200mg/ml twice weekly. My T came back 839. I'm going to find a new dr but I need to start an AI or I'm gonna lose it. I'm wondering how long lowering e2 would result in alleviating mood issues assuming of course I don't overshoot to low.

A little scary to take an AI in the absence of good labs. Why 240mg of T? That's a whole lot and is much more than the normal TRT dose. Lowering the T to more average amounts would drop your E2 and you may not need an AI at all.

Im gonna try and see him today to ask why 240, but if it resulted in an 839 blood level then perhaps he made a lucky guess? Or if I lower dose is it possible I keep same T level and just lower e2? I've read a bunch of posts from people who went up to 240/week to get adequate bloods.

Anything is possible, but I'm wary of your lab results. Most guys injecting 240mg would have peaks above 1000 easily. I inject 80mg per week and my trough is just slightly lower than your peak!
 
ya you really need to move on here. That's probably the best advice. If you got some AI use it just long enough to find a new provider and start over, as it were.
 
I wonder if you did need an AI, I don't think you're estrogen levels are high enough to cause emotional issues. Probably not something you want to hear.

My friend Vince Carter is right - time to head for the door and find a new doctor. Too many lab tests missed, too many unanswered questions...you deserve better.

No I have no problem hearing it but I'm surprised you dont think 77 was too high. Of all the blood test results I've seen on this and other boards, 77 seemed pretty far in the high range.

We typically look past what the number is and work around symptoms which you haven't said anything negative about that reading...moodiness, irritability, overly-emotional, night sweats/warm/hot, reduced libido, etc etc
 
I hear you on the "high" e2 and symptoms with moodiness, etc....
I get somewhat symptomatic myself with anything over 40 pg/ml so it's probably not all in your head.
BUT.....................as the others have said, you do most likely need to get a better doc to work with you on this.
That protocol doesn't seem to be helping you.

And if (and only if) you end up needing an AI, then I noticed results very rapidly, as within a few days to be honest.
Just don't go rogue on this.

If he doesn't understand the relevance of testing for SHBG, then he lacks the knowledge and experience to balance your hormones. It typically takes 24-48 hours for your levels to peak, so testing 18 hours after injection is stupid! You're likely peaking much higher 24 hours after your labs. I doubt he will even prescribe you an AI, for the fact he doesn't understand why SHBG is so important I expect him to tell you AI's are for female breast cancer patients and it's not standard of care.

Systemlord is right. Absent an SHBG value, the protocol designed is a guess.

Forgot to mention your doctor likely ordered the wrong E2 labs, if he's clueless about SHBG testing, he's clueless about the proper E2 sensitive LC/MS/MS method typically used if you're not a female. So your E2 labs are likely overstated using the wrong test, I'm saying your E2 is likely lower than stated.

We've all been in a very similar situation.
