Whenever I've taken either Ibuprofen or caffeine (Ibuprofen more so), libido & frequency of erections shoot right up. I'm not taking any exogenous hormones right now & I have low LH and low DHT.
Does Ibuprofen and caffeine increase LH or DHT?

dang man! I think I am having an epiphany. The last time I had a rock hard erections I was hungover. The night before I took some Aleve. That's the last time I took Aleve and the last time I had great erections.

Then I see this post from Madman about how Aspirin can improve ED. It may be time for some experimentation.

I can definitely tell when I am on aspirin daily my glucose pricks have the blow flowing like crazy! I am sold on it for better blood flow.

Aleve is Naproxen, not Ibuprofen. I don't know if I would get the same effect from Naproxen. I certainly didn't get any positive effect when I used Diclofenac. For me, there's something about Ibuprofen that increases both libido & erection frequency.
I found the article below which refers to a study done that found Ibuprofen didn't increase testosterone, but did increase LH. I think that LH is very important for libido, which explains why so many guys on TRT experience either no increase in libido or worsened libido.

Ibuprofen is a RHO-Kinase Inhibitor. It works on erections on a different metabolic pathway than does any other NSAID... or PDE5 Inhibitors (Viagra, Cialis, Levitra Stendra, etc.).

After surgery for an umbilical hernia I was taking ibuprofen, alternating with acetominophen. I definitely felt more aroused but my BP spiked as did my blood sugar and I read that ibuprofen is implicated in both. Ray Peat has written that aspirin inhibits aromotase. As for caffeine, I'd have to observe when I'm drinking tea; coffee spikes my adrenaline.

I've had the what I thought unpopular opinion that low dose ibuprofen is somewhat of a wonder drug. In my case half the recommended dose before bed helps my sleep quality dramatically with no apparent downsides. I've heard others talk about how it cured their prostate related nighttime wake ups to urinate as well. Now this. Interesting.

As I posted, my reaction to ibuprofen in regards to BP & blood sugar wasn't good, but the lab rat in me wants to try it again! IF it improved erectile function, maybe I'd take it now and then. My wife, after years of indifference to sex, now is being playful, though this could be related to dementia, which I've read. If so, it could be short lived or she could do a 180. I guess I should give it a try while she seems in the mood.

That is very interesting! I have done some reading on the Rho-Kinase pathway with regard to it being one of the pathways that maintain the penis in a flaccid state. I did not know Ibuprofen was an inhibitor of Rho-kinase.
Altered RHOA/ROCK activity in the penis is being studied as a potential cause of ED.
I have noticed this effect with ibuprofen as well in the past.

Took 600 mg ibuprofen yesterday afternoon. Nothing. But, when I was taking it after my surgery, several times per day, I did have a higher libido/desire, though not corresponding erectile improvement.

Is there a medicine that does this without all the unwanted effects of Ibprofin? Or supplement
That's what many of us are seeking. The only one(and I haven't tried it)that has a long history in Traditional Chinese Medicine is horny goat weed. Horny Goat Weed: More Than Just a Name | Medicine Hunter

It's mode of action I've read involves inhibiting PDE-5 but slowly. I've also read that the concentration needs to be 40 to 50% icariin, but that's from companies selling a finished product, though the article states 10% icariin. Since its been used in China for 2000 years, no one knew about the amount of icariin. They just let the leaves steep in hot water for probably 20 or 30 minutes and sipped it or drank it, maybe several times a day. There's also ginseng; panax and Siberian, though there are other varieties, IIRC. Panax is stimulating. Siberian is more like a tonic that 'tunes up' the body.

I was responding to the second question with the link to the article on horny goat weed. I don't know if its actions include acting as an RHOK inhibitor but with herbal supplements, its benefits could be from a broad spectrum of properties, IMO.
