I'm happy to be here with a group of experienced T replacement users! Glad to have found your Forum

I'm a 79-year young professor teaching physicians how to be more experienced teachers in Germany half the year (every two months) and in NH the rest where I write, fish, and play. My NH Urologist tested my T three times, once originally after my complaining about some ED issues and then twice more over a 3 month period including after Transdermal 1.2 mg/24 hr patches for 6 weeks in Germany and it was always under 300. He did other tests to rule out petuitary issues, etc; and then I left for Germany where I began using 2.4 MG "Testopatches" (I need no prescription for those in Germany) and also got myself some Andro Gel 50 mg and will not see him again to ask him to raise my T dose until in August when I return again.

After the first 4 weeks use of the patches, I noticed a significant increase in libido (masturbating or having sex with my much younger wife once a day or so) but still am wrestling with ED at least 50% of the time. I use Cailis (5mg/day normally for BPH increasing it to 20mg when I expect to have intercourse).

1) Since the 2.4 mg patches every day do not seem to either raise my T to 500 or more, and since I'm still having some ED issues, does it make sense to still use the 2.4MG patches PLUS a daily treatment of the Andro gel massaged into shoulders and thighs? What about using two patches plus the gel? Is that too much?

2) What is the negative issue, from what I read, against massaging the gel directly into testicles and penis where I understand it gets into the system faster?

3) I read that T treatments cause a shrinkage of the testicles. Is this true with the gels and patches as with injections? Can one take a pill or something to counter this? (I'm not yet ready for the injections but will if you think it will make a significant difference in my ED issues.)

Though these supplements might help me (my friend has considerable empirical evidence of their positive effects, though these studies are often secret and not always publicized), having a younger wife is what seems to help the most along with coming from a context of being 45-years old instead of 79 in all that I do including 30 minutes of vigorous exercise a day and being a hunter and fisherman. But it is the ED I'm most interesting in helping with the T, though I know it should also help me build muscle and bone and have higher libido. Should I inject HCG or use other things in conjunction with my T replacement? What do you think of changing to the injections and what dose?

Your forum and all the information, much of which I have browsed and some of which I have read, is an invaluable source of information and I readily see there is far more knowledge here than my urologist seems to know which does not surprise me.

I will try to answer your questions. At the end I will also provide a link to the most important posts that everyone should read. You can also feel free to post your blood work in that folder under this main section.

What about using two patches plus the gel? Is that too much?

Why not just use the gel alone (do you have enough?) and try 7.5 grams per day (assuming 1-2 percent). Yes, you should strive for a total testosterone over 500 ng/dL (17.3 nmol/l) and a free testosterone of over 2 percent of total. The dose of T should be adjusted to achieve this goal. Be careful with high hematocrit since this is a side effect that affects older men a lot more than the young. Again, we would love to comment on your blood work.

The scrotum is a great place for T to DHT conversion, so your DHT will be higher (Higher DHT is associated with higher sex drive but also to more side effects like acne, hair loss and possibly BPH). Also, some gels like Androgel burn that tissue due to their alcohol content. How is your current DHT?

Yes, HCG works great to bring testicles back to baseline size and to boost sex drive. I use 500 IU twice per week along with TRT.

I understand your concern with ED and many men don't find that Cialis or Viagra work very well.

A sure fire bet is the use of a drug called Trimix.

I can only tell you it will give you an erection like a 16 year old again and your wife will be a very happy women...trust me on that as I know first hand

Thanks for your counsel. I did not get my lab tests from my urologist but will ask for them when I return to the US in August and be happy to share them then.

I like the idea of using the gel alone. But I have a month's supply of the patches still so could use them in conjunction with one of the 50mg containers for Andro gel. Not sure how you get to your suggested 7.5 grams per day? What tests will monitor for he high hematocrit?

I do not know what my DHT level is. Again, what tests do I need for DHT levels? I guess I have to educate my urologist.

I tried the androgen on my testicles and penis and felt no burning, but you say I might get more DHT from using it that way. So will be careful.

Regarding the prevention of testical shrinkage, can I get HCG in a cream or other form than injectable?

I read through all your suggested web readings.

I tried the Trimix, (also MUSE pellets which do not seem to work), as suggested by Gene Devine below, several years ago but wanted to try to preserve the full strength erections without having to inject, if possible, and also hoping for longer possible erections since my sexual play is not on a schedule so was hoping the T along with Cialis would work. Maybe when my T is higher than 300 this will happen. I see the delicate balance between T and DHT levels is important. Now I have boosted my libido from the T replacement therapy, but need the quality of erections and their sustainability to follow. (By the way, though I teach in medical schools, my doctorate is in psychology and I'm not a medical doctor.)

Gene, thanks for your thoughts. I have tried Trimix 20 years ago and am hoping to achieve what it offers, as in my response above, without the injections as my sexual play is not that schedual-able.

But that's just what I want: erections like I had when I was 16!!

Well, for me (and other men as well) TRT didn't have a major impact on erectile quality but it did libido. I can get an erection but just can't hold it as long or as hard as I used to and PDE5i's only provide some help but not great.

My sex is somewhat scheduled but the injection is just so easy, fast and painless I can't imagine not using it when we want a hot longer sex session.

I just love the stuff as I said and so does my wife...maybe more in fact;)

I've had the same experience Gene. TRT improved my libido, but can't say it's done much to improve the quality of my erections. I too seem to achieve an erection fairly easily, and will sometimes lose it during sex. I find if I can go without for 24 hours, the next day I'm pretty hard.

First, you really need to do a trial in the office so the Doctor can see if the Trimix is formulated correctly to give you the erection that will give you the result you are looking for.

It's a bit awkward no doubt but get over it as the long term results are worth it!

You need a prescription for Trimix and I have never seen a place where you can buy it without.

The cost is about $20 per injection; the same for a 200 mg of prescription strength Viagra.

I'd pay more if I had to as Trimix is the real deal and changed my sex life in a great way.
 
I did one of these "awkward" trials 8 years ago in a urologist's office. Was hoping to avoid the time needed for appointment ( months...) and trouble in order to get it soon as my lady visits Sept 1st....

Depending upon where you live there may be ED Doc who specializes in Trimix. I know where I live there are at least 3 to 4 clinics that do nothing more!
