Hematocrit and platelets high (while on blood thinners and off TRT for 3 months)

I don't believe my new-ish membership let's me post pictures yet but luckily it's only two values that "worry" me. My hematologist has quite the wait list, so I thought I'd see if I could get some info in advance.

Long story short, diagnosed with mesenteric and portal vein thrombosis, halted all TRT (for 3 months now) and went on blood thinners (enoxaparin for a month, then two months of xarelto). The following values are WHILE still on xarelto:

hematocrit 51.6 on a scale of 34-47
platelets 598 on a scale of 130-500

Hemoglobin and RBC are at the exact max, so they don't trigger a "high" warning on the labs, yet they must be related.

Not to mention my d-dimer is 745 on a scale of 0-400 (a healthy individual, or someone on blood thinners should have a zero score).

I know this is all quite advanced, I was hoping maybe someone has had similar issues before and can provide some more insight.
