Gyno and My E2 Levels - When to Worry?

What are the typical estradiol ranges where someone will see gynocomastia? I have always had a bit of it since I was a teenager, however it appears to have gotten worse since beginning TRT but my estradiol levels seem to be low. Below are my latest lab results.

October- Dosage 70 mg test cyp. injected twice weekly (140 mg total) and .125 anastrozol twice weekly.

Total Test- 1130 ng/DL Free Test- 136.74 pg/Ml Estradiol- 37pg /Ml SHBG- 48 nmol/L

I wanted to increase the frequency of injections to every other day to help with aromatization. My SHGB has always been on the high side and so my physician has said we should look to increase my free test. My physician recommended 50 mg test cyp. every other day which effectively increased my dose. During this time I experienced what I thought were high estrogen symptoms (gyno getting worse, nipple sensitivity) so we increased the anastrozol to .125mg 3 times weekly. Below are my latest results.

Total Test- 1435 ng/DL Free Test- 268.3 pg/Ml Estradiol- 13 pg/Ml SHBG - 32 nmol/L.

I'm wondering if maybe I am just experiencing high testosterone symptoms and mistaking them for high estrogen. I have noticed water retention lately and increased acne on my shoulder since the change.

Any insight/input would be greatly appreciated.

Thanks

Whats the Gyno history? YOu have diagnosed Gyno or you think you do because of some puffyness or other nipple issues? Gyno in guys on TRT would be very unlikely unless there's a clear defined history; genetic.

Your dose is way too high, 50mg EOD is way too much, your Free T is way too high, E2 is probably way too low, need to see the lab ranges to determine if you even had the right testing performed. 37 even on the previous protocol was by no means "high".

I never have Gyno officially diagnosed, but since adolescences I've always had fatty issue under my nipple area no matter how lean I get. Below are the lab ranges.

Total:250-1100 ng/dL
Free:35.0-155.0 pg/mL
Estradiol: < OR = 29 pg/mL
SHBG: 10-50 nmol/L

I have a consult with my physician on Monday and I believe we will scale back the dosage. When I was doing 70mg twice weekly 140 total my levels seemed to be within a high normal range but like I said I wanted to increase the frequency of injections. Cutting it back to 40mgmg EOD should average it out to the same as 140mg per week unless I'm doing my math wrong.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
