Several studies have reported decreased serum DHEAS levels in patients with ED and denoted that decreased secretions of DHEA and DHEAS are important risk factors for ED in aging men [41]. DHEA levels reach their peak in the third decade of age in men. The serum DHEAS levels were significantly lower in the younger patients with ED compared with non-ED patients. Diminished DHEAS levels, especially in young men with ED, may either be an etiologic factor for ED or a negative consequence of it. However, patients treated with DHEA had a statistically significant increase in all domains of the IIEF in contrast to the placebo group. Interestingly, the serum DHEAS levels increased significantly after sildenafil citrate treatment in the ED group (especially in patients younger than 50 years). Also, patient age was an important factor affecting the sildenafil citrate response [42].

The first clinical effects were reported after 8 weeks of DHEA treatment and a remarkable improvement in maintaining the erection after 16 weeks. Prostate volume, postvoid residual, prolactin and PSA were not significantly changed after DHEA therapy [34]. Despite the evidence that has questioned the validity of improvement in sexual function when treatment relied solely on DHEA administration [43], several previous studies had demonstrated the usefulness of DHEA in treating sexual dysfunction associated with testosterone or DHEA deficiency [34]. A nice explanation of that effect was demonstrated as both testosterone and DHEA are considered central neurosteroids and have been found to modulate endothelial function, which ultimately influence libido and penile erections [44]. Several studies on other organs support the protective role of DHEA and DHEAS i.e., reduction of vascular stenosis in heterotopic heart transplants [45]. Because there is also some experimental evidence for direct effects on the brain, the possibility of a central effect of DHEA might be evident. An interesting mechanism that could better define the role of DHEA in sexual performance is that administration of DHEA results in biosynthesis of active androgens by tissue targets without representation in the peripheral circulation [10].

Although experimental studies had demonstrated that DHEA has direct genomic and nongenomic effects on the vascular endothelium, however its clinical effect as a facilitator of penile erections is not evident [46]. Most of the literature had not confirmed the role of DHEA in the field of sexual medicines [43, 47]. Additionally, other studies questioned the concept of DHEA as a neurosteroid with a central putative effect on libido [48], although this hormone had been found to influence self-esteem and mood in younger individuals [49]. Furthermore, the evidence-based studies supporting an anti-aging, anti-inflammatory, antiatherosclerotic actions, and antidepressant effect of DHEA supplementation are often poor [50, 51] (Table 1).

I find it hard to believe that low levels of DHEA would give someone ED, and supplementing with DHEA will correct this problem. When I tried supplementing with DHEA before I was on trt. It gave me ED.

My estrogen is a little low so I hope supplementing with it will raise my e2 into the 20’s without lowering my testosterone. My e2 is at 17 right now but I hope that dhea can fix my mild E.D. by increasing e2 a little

1) Boosted Libido.
2) Turned me from a grower into a shower.
3) Turned on nocturnal and morning erections - which had been absent for at least two decades.
4) Mostly cured my stubborn ED. Previously I could not get erections unless I used trimix. Viagra and Cialis did not work - even if I took 3 or 4. After DHEA I get erections naturally every day or every other day.

Everyone is different but for me it has been a game-changer. It was like a light switch - that is, changes began with a day or two. I take 25mg per day.

What brand did you use? It sounded like it boosted testosterone a bit but I thought dhea does not do that?

LifeExtension 25 mg. Got it from Amazon.

I honestly don't know why it worked I just know it did. Labs didn't seem to change much but life sure did!

Past evidence documented a possible role of an agedependent decrease of DHEAS in the pathogenesis of ED.42,43 The MMAS found that DHEAS was the only hormone negatively correlated to the prevalence of ED among 17 investigated hormones, including T and E2.91 In addition, Basar et al,92 in a consecutive series of 348 men, reported that DHEAS and free T levels were significantly lower in men with sexual dysfunction as determined by the IIEF score. However, more recent data have advocated against an association between DHEA (and its DHEAS) and ED. A large placebo-controlled RCT evaluating the effect of DHEA in elderly men and women with an observational period up to 2 years failed to demonstrate a physiologically relevant beneficial effect on ED.93 Similarly, Morales et al94 reported that DHEA treatment (50 mg twice daily) did not induce significant differences in sexual performance outcomes compared with placebo as assessed by the IIEF, the Androgen Deficiency in the Aging Male questionnaire, the Aging Male Symptom Scale, and the Global Assessment Questionnaire. Similarly a meta-analysis evaluating the effect of DHEA administration in elderly men indicated that DHEA supplementation did not improve the overall score of the IIEF-15 or EF domain total score.45

The prevalence of ED in patients with Cushing syndrome is unknown. In particular, no information has been reported in the European Registry on Cushing’s Syndrome.47 Granata et al44 found that in subjects with adrenal insufficiency, treatment with glucocorticoids and mineralocorticoids independently improved the EF domain of the IIFF.

RECOMMENDATION 10

DHEA and DHEAS are not involved in the regulation of male EF (level 2A).

Glucocorticoid and mineralocorticoid in adrenal insufficiency might play a role in restoring EF (level 4C). Prolactin

Yes madman I would not have expected DHEA to have the effect it did on me and according to my labs I already had high DHEA levels. The next guy could take it and have opposite effects. Go figure.
 
I know this thread is a few months old but I am glad to see this post. I have been on a stable TRT protocol for quite some time but have recently been experiencing ED (and lack of morning/nocturnal erections) despite no changes to my protocol, which includes 5mg daily Cialis.

My 6 month labs came back and the only thing that stuck out was low DHEA (which has never been low) at 130 out of a range of 138 - 475.

Could this be a contributing factor? My follow up appointment is a couple of weeks out so I’m just trying to gather info in the time being.

I know this thread is a few months old but I am glad to see this post. I have been on a stable TRT protocol for quite some time but have recently been experiencing ED (and lack of morning/nocturnal erections) despite no changes to my protocol, which includes 5mg daily Cialis.

If you try supplementing with DHEA. Let us know if it makes an improvement in your ED. Hopefully it will cure the problem.

The only time I felt anything DHEA, was before I was I was on testosterone. It did give me ED, it was converting my testosterone into estrogen. Now that I'm on trt, I wonder if I double my dose what affect it would have on me.

DHEA has a very positive affect for me, perhaps moreso that TRT in the sexual function area.

It has a similar effect on me. It is the one thing that turns my sexual function on. Also nocturnal erections. After two decades of having no nocturnal erections, they returned within 1 day of supplementing with DHEA. I take 25 mg. daily.

I have noticed the same thing as Leesto but not to such an extent. Morning wood increased and spontaneous erections occurred in the first week of taking DHEA supplement (100mg). My test from the endo showed my DHEA level was 383. That is actually above the range for my age and my T is on the low side (451 measured in the A.M. ) . However I have noticed less of an effect recently ,taking the DHEA. It's almost a month now. Cycling is probably what is needed.

Everyone has a holes in there hormonal balance. I can see how dhea can benefit. Jsut as HCG helps others.

I was on dhea daily and my levels spiked after 39 days. Scalp and skin for very itchy. I stopped and it went away. Now I take it eod.

What’s a good dose? Is the itching really a side of to much dhea?

That’s what I’m trying. I’m at 18 pg and 400 total test. Just finished an Enclomiphene pct at 25 mg per day to try and increase my test levels. Will test bloods in 6 weeks. If no change, I will try hcg followed by regular Clomid for 4 weeks. And if that doesn’t work, I will probably try trt.
