IMPACT STATEMENT

Measurement of free hormone (FH) concentrations in biological samples presents a challenge to the clinical laboratory. FH concentrations are generally very low, requiring use of sensitive and specific techniques. Furthermore, special attention must be placed on the equilibrium between free and protein-bound hormone when separating and analyzing in vitro. This review will enhance the readers’ understanding of the current state of mass spectrometry-based methods for the measurement of FHs. The advantages and disadvantages of different separation techniques and sample preparation methods are discussed, as well as clinical conditions in which measurement of FH is warranted.

Yes there is such a thing as AR DDS (distribution, density, sensitivity/polymorphisms).

When it comes to sensitivity of the AR, polymorphism of the AR/CAG repeat length (long/short) tread lightly when you speak on such especially those pushing that everyone and their brother needing to run these very high/absurdly high trough FT levels 30-60 ng/dL in order to experience the beneficial effects of T!

Yes we are talking TROUGH f**king levels here!

Yes there is also such a thing as running too high FT!

it will be interesting as time goes on if they come up with standard free T ranges.. peter atia says no one has done a good job on coming up with proper free T range/test/interpretation chart.. may be better used for TRT vs diagnosing hypogonadism would be my intuition.

it makes sense but also I think endos know there is likely other things that bind test up so focusing on it only isnt super helpful either. of course there is also the fact that bound up test is also a GREAT thing and acts like delayed release so super important.. the downside is also bound is mor likely to be expelled as waste. of course basing off of symptoms is hard too as everyone wants more energy or starbucks and monster energy wouldn't be billion dollar companies. also everyone has anxiety so another tuff one.

Also some of these DRs aren't being intellectually honest with there charts.. is being overweight causing lower T or at what point is low T causing weight gain? what about chinese people who are small and have low T? what about african americans having HIGher T but ALSO obese BMI? so if they want to be taken seriously by critical thinking scientists/DR peers they need better graphs ;) or are they saying overweight people have low T vs Low T causing obesity?
