1st bloodwork and have questions

Naturopathic Doc does my TRT.

My currently protocol for previous 6 weeks:
Test Cyp 100mg E3.5D
HCG 500iu E3.5D
Anastrozole .25mg weekly.

Doc said to give blood (specify that I want to donate platelets) and to increase Anastrozole from .25 weekly to 1mg weekly.

Can somebody please take a look at my blood results and please let me know if there is anything else you would recommend. Test was taken on injection day, (prior to injection). Thank youPrioir to TRT

I would lower with T dose to 50-75 mg twice per week.

No, I am not taking anything else. Libido was high during week 2.5-4. Now its low again. Doc said that its probably due to high estrogen. I want my libido back and so does my wife.

So you think I should lower my test dose down to 50-75mg E3.5D and leave anastrozole at .25 weekly?

Your doctor did not run the correct estradiol test. The only accurate test to measure estradiol in men is the assay that determines the value via liquid chromatography/dual mass spectrometry.

That doesn't surprise me. Based on the test that was used, do I add or substract 10pts to give guesstimate

Subtract 10, but know that is a very rough estimate only. Based upon that though (or even based upon the original test #), your E2 is low, not high. Your T/E Ratio is: 1500 / 45.4 = 33. Ideal T/E Ratios are between 14-20. Low E2 kills libido as fast or faster than high E2.

I would not add any Anastrozole if I were you unless you have overt high E2 symptoms like itchy/painful nips, flushing, night sweats, etc...In fact, I would stop Anastrsozole altogether, wait 4 weeks with no other changes, and then test again with the sensitive assay.

Interesting. The only sign of E2 is puffish nipples and more emotional watching movies, No bloat, Sweaty while sleeping but nothing that my wife noticed and Low libido.

I am afraid to stop Anastrozole but willing to do so if needed.

Self-test: the sensitive estradiol assay is available through Discountedlabs.com at a reasonable price. Understanding your symptoms, how your body is responding to your protocol, is critical and you do seem well self-aware in that regard, but it's foolish to make adjustments without proper labwork to guide you. As my doctor says, changes without labs is akin to driving in the dark without headlights.

Puffish nips and being more emotional *may* be signs of a higher E2 level than your body likes, but as CoastWatcher said, get your own test and then no more guesswork.

He moved from TT @ 192 pre-TRT to over 1500 on TRT, feeling great for the first few weeks and then libido suddenly disappears, and you guys suggest he might have low E2????

Might be that some people could more sensitive to E2 than others? The same way some people only feel the benefits of TRT at over 1000 while others feel great at "only" 700?

Absent a sensitive estradiol test, it is impossible to determine what his levels may be. Hence, my suggestion that he obtain the results via self-testing and determine with his doctor where adjustments should be made.
