No Results on TRT—Adrenal Crisis? Low Cortisol & need HELP!

My body has been wrecked since 2020. (got COVID19 June 2020, then hit by 2 hurricanes, August & October 2020). I am not sure if COVID was solely responsible for my current issues or the mix of two high stress environments caused me burn out to the point of adrenal failure which is what I believe I have.

Symptoms (Screenshots):
-Low AM Cortisol (5 out of 6 different test)
-Low Androstenedione
-Low FSH
-Low LH
-Low PSA
-Low Libido
-High T3
-High Blood Pressure (never before 2020)
-High Bad Cholesterol (never before 2020)
-High Triglycerides (never before 2020)
-Get Dizzy in mornings when I stretch really hard or stand up fast after kneeling down (never before 2020)

Background:
I am a 33 year old businessman who has always HAD a ton of drive, get up, energy & great memory….until 2020 (I was 27 then).
In 2020, my memory, drive, energy—everything basically collapsed.
I went down to 30% potential—Hard to get out of bed. Some good days but more bad days. Random panic attacks would hit me during high stress situations (tight throat & tight chest, sometimes I’d even break down & cry which is unheard of for me)

By early 2022, I didn’t see any improvement of my body self improving so I took a path to find help.
—Went to Amen Clinic & did the SPECT Brain Scan—everything healthy no problems.

—Told my PCP, who I was seeing regularly due to always being healthy, and they prescribed me Adderall. It helped, but felt that it was not the solution.

Finally in late 2022, I was referred to an endocrinologist who got me to begin TRT due to my Free Test at 6.29ng/dL & Total Test at 223ng/dL. I had never gotten my hormones tested prior back when I was in a healthy state for a baseline.

Now, In the 3 years I have been on TRT, I’m maybe at 60% of my potential. Still battling the same stuff prior to TRT just not as often, but it is there. My memory & drive is still trash tbh.

I finally signed up for FunctionHealth.com, & I tell everyone to consider getting this. Which is when I found everything no doctor or endocrinologist has ever ordered or looked at (to my knowledge)

Now, I bring this all here to share, as I am seeking advice on what I should do?

I can tell you what works for me. Eat non-starchy vegetables, meat and eggs. You'll be surprised how good you will feel. I shouldn't forget drink water and if you drink coffee make sure it's only black.

I’ve been doing all of what you suggested, just without the eggs/dairy as much & no coffee.

After doing a load of research, I am questioning if I should even be on TRT given I’m consistently low in Cortisol?

I weaned my TRT dosage down from 1/2cc (200mg/mL) twice a week to 1/4cc (200mg/mL) twice a week—basically just cut it in half. Did not experience any differences, not better or worse & I ran that for 8 weeks.

After this, I moved to stop TRT altogether for 4 weeks, then took a blood test, this lab result reflects on what I updated (the furthest right side dotting).
 
Get bloods for: Ferritin, TSAT, TIBC, vit A, Copper, Zinc, **Pregnenolone**, Glutathione, igf-1.

Have you spoken to your doctor about having imaging of your adrenals/pituitary?

Also based on what you've shared, possibly GnRH &LHRH Stimulation Tests.

Honestly, you should be getting Pituitary & adrenal imaging done. The fact that with those levels you haven't been prompted to do so is fucking wild.

Pregnenolone is what the body uses to make testosterone, estrogen, progesterone, cortisol & is produced in various areas(primarily the adrenals & glial cells in the the brain)

LH & FSH are stimulated by the secretion of GnRH, which itself is created by Kisspeptin neurons in the hypothalamus(Kisspeptin & Neurokinin B primarily)

Appreciate the feedback & context. TBH I don’t know what I don’t know. I am more and more alarmed and judging myself as the time continues to pass and I find myself not actively pursuing solutions.

What is crazy is the level of difficulty one faces when their levels are completely jacked up. The lack of energy, drive and feeling of overwhelm does not pair well with hunting for solutions—especially after finding no closure between various endocrinologist, hormone therapists, functional medicine docs, etc, but then the fire from under comes back and you start again. Thats where I am now.

I do plan to ask my next endocrinologist to request imagery of my adrenal & pituitary glands. I am a cash pay patient so it’ll be fun getting that bill, but if I’m successful in getting a solution which allows me to perform/operate my company like I was 5 years ago, at age 27, I know I can make everything invested in my health back in reasonable time.

My goal here is to read feedback from others and hopefully one other unlucky person out there, like me, has walked down the same road and found a solution which I can take up to my next appointment to have the doc prove or disprove that as a remedy
