TRT Low SHBG Protocol change confused Sustanon 250 Australia

Hi Guys,

I have been prescribe by endo in australia for TRT original back in November 2018 Sustanon 250mg. After a lot of reading i was worried about my SHBG being so low. I do have full bloods thyroids etc.. all are ok in range just want some advice on my T, Free T and E2 levels moving forward. My age is 34, 2 kids never done AAS just been in the dumps for many years so wife told me to see the doc which lead me onto TRT.

Pre TRT:
LH: 2 (2-8)
FSH: 2 (1-8)
Total test: 2.5nmol (6-35 nmol)
SHBG: 12 (15-50)
Free test: 72pmol (200-725 pmol)
E2: 89pmol (<150)

Starting TRT i was originally prescribed sustanon 250mg 1ml every 14days i had real bad E2 issues bloat, and anxiety rushes few days post injection and basically started to bottom out around days 8-10.

8 weeks SUS250mg 1ml every 14days TRT: bloods taken on my trough day 14. Also 500IU HCG twice per week
Total test: 10.7 nmol (6-28 nmol)
SHBG: 11 (15-50)
Free test: 338 pmol (200-725 pmol)
E2 Sensitive: 210 pmol (50-150pmol )

So doctor prescribed me anastrozole start with 0.25mg twice per week which i haven't taken i done some research about low SHBG and requested to move to every 10 day injections 250mg 1ml with option of 0.5ml 125MG Sustanon 250 every 5 days which he agreed.

6 weeks later SUS250 0.5ml every 5 days TRT: bloods taken on my trough day 5. Also changed my HCG 250IU EOD.
Total test: 26.1 nmol (6-35 nmol)
SHBG: 25 (15-50)
Free test: 682 pmol (200-725pmol)
E2 Sensitive: 148 pmol (50-150pmol )

So as you can see im pretty stoked with the new results i managed to get my E2 down without any anastrozole and my trough levels in range. Doc wanted my trough to be between 20-30nmol as my peak will be higher also E2 few days post injection.

What i can't work out is how the hell did my SHBG goto 25? from 11? I have been doing my head in about low SHBG and basically i think i have near perfect SHBG now for TRT and free T still in range. Any ideas on this? Was in less T more frequent which raised my SHBG? Or was it my E2 decrease which raised this?

How do i feel? bloody fantastic zero estrogen signs dont feel bloated feel great to be honest its still a work in progress my next follow up appointment is in May with more bloods to follow then. I feel the past 4 weeks of my 6 week protocol change have been best so far.

Thyroid reference ranges aren't normal, some of the group of people that made up the ranges were later found to have thyroid dysfunction, so thyroid labs in range is no guarantee your thyroid is normal. Add to this doctors can't agree on reference ranges for thyroid hormones and is still under debate.

Insulin resistance and obesity can lower SHBG, so if you lost weight and saw a reduction in insulin levels, this could explain an increase in SHBG.

I have SHBG on the lower end as well (22), I do much better on multiple smaller injections per week than I do on larger less frequent injections, most low SHBG men do best on more frequent dosing as it keeps levels more steady which is important if say estrogen is dipping to low or a little high at peak 3 days after an injection.

I haven't lost any weight besides maybe 1kg water from the drop in E2 between protocol changes i weigh 100KG 5"11. Solid frame gym 6 days a week past 2yrs weights and 2 days cardio. BF is 22% so not slim by any means. Trying to get this down diet + cardio.

Here are my thyroid panel + IGF1

Free T4 17.7 (9-25 pmol)
TSH 1.88 (0.50-5 mIU/L)
IGF1 30.2 nmol (9.3-30.4)

I have also been on HCG 250IU EOD since new protocol change to every 5 days. I used to be on HCG 500IU twice per week at the start on every 14 day protocol.

I have had about 6 blood test done since Nov 2018 and my SHBG pre TRT always between 10-12 pre TRT. its only gone up now since protocol change is all i can pin point?

I do want my E2 to come down a little more i'm planning to recheck this again in 3-4 weeks time hope it settles down a little more. If it doesn't i was thinking to go every 3.5 days twice a week shots ran this by the doc first. Roughly im on 175MG per week at the moment which works out to be 125MG every 5 days out of a 250MG 1ml ampule at 10 day mark.

My plan would be if E2 doesn't come down 0.35ml (87.5mg every 3.5days 175mg per week of sustanon 250mg prescribed every 10days)

Normally we see SHBG decrease after TRT commences, on rare occasions we a select few who see SHBG increase. Losing weight should lower estrogen since men convert testosterone to estrogen through the aromatization process within fatty tissue, so lower the body fat and lower the estrogen.

I wouldn't change anything since you are feeling so great, don't play the numbers game like a lot of men do, go based on how you feel.

ok noted. thx systemlord appreciate your time. All the reading on forums i see exactly what your saying SHBG gets knocked down a fair bit. i was fairly depleted and lacking T for a while maybe it was a wake up call to the body maybe i had mid shbg level who knows only guessing.

hopefully E2 comes down next 3-4 weeks bloods before my doc's appt. and shbg stays stable hope it wasn't a lab error either time will tell.
