* you only need testosterone levels up for about 4-6 hrs in the day to produce an effect which works through the androgen receptor, the androgen receptor is present in every cell in the body so its not just a sex hormone it is a health hormone

Another powerhouse in the field!

* He presents compelling data showing that Testosterone Replacement Therapy (TRT), when bringing levels back to the normal range, can significantly reduce all-cause and cardiovascular death rates, improve quality of life, reduce insulin resistance (the core defect in diabetes), and even improve cognitive function, advocating for its wider adoption in clinical practice as a "health hormone."

Hugh Jones's presentation at the Longevity Forum Science Summit delves into the strong, often overlooked, association between low testosterone in men and increased cardiovascular mortality and chronic health issues. Drawing on research from Sheffield and other international studies, Dr. Jones highlights that low testosterone is not simply an aging factor but is highly correlated with functional hypogonadism caused by co-morbidities like Type 2 diabetes, obesity, and metabolic syndrome. He presents compelling data showing that Testosterone Replacement Therapy (TRT), when bringing levels back to the normal range, can significantly reduce all-cause and cardiovascular death rates, improve quality of life, reduce insulin resistance (the core defect in diabetes), and even improve cognitive function, advocating for its wider adoption in clinical practice as a "health hormone."

So, is he advocating gel use? Many seem to really get a dip in energy levels with gel or cream in the evening, i believe the fluctuation is bigger with once daily gel/cream than natural. Too bad about the audio.

No he was pointing out that T levels do not need to be high all day long a several‑hour window (4–6 hrs) of adequate levels is enough to activate ARs and trigger downstream biological effects.

Simplified way of describing the pharmacodynamic idea that receptor binding and downstream gene/protein changes can be sustained even when serum levels are not continuously high.

This underlines why TRT protocols can aim for physiological peaks rather than constant supraphysiological levels.

This is why very fast acting Natesto dosed 2-3X daily or fast acting oral TU dosed 2x daily (morning/evening) or in many cases using Dr. Suns hybrid protocol (morning/noon) are effective.

It's a myth that you need to have your trough FT level jacked up high/absurdly high 24/7 steady-state!

Abraham Morgentaler hitting the nail on the head here!

Pros/Cons oral TU (51:24-57:12)

* one of the things that the orals have transformed is the concept that you have to have a continually high level of testosterone to get the benefits and clearly that's not true and the safety profile seems to be improved by having levels that fluctuate some during the day returning to close to or even baseline

Yes, Oral TU is not available in Europe, been thinking of combining IM TU with a small dose of gel daily, since a transdermal alone applied once daily usually ends up taking levels below baseline at through.
It's just a matter of getting a prescription for both at the same time and i will try it out.

I think everyone here would agree that using oral test to boost levels is better than nothing. And sure, you don’t HAVE to maintain high levels constantly to get benefits from trt. But at the same time, there are numerous studies that show injectable testosterone provides more health benefits and symptom resolution than oral test, or other fast acting forms like gel or cream. And yes, a case could probably be made that some of those benefits come at the cost of an increased risk in other aspects(like hematocrit). But I’d also say it’s not that difficult at all to use injectable testosterone in a safe and effective way that will provide more benefits than oral testosterone.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
