Hello all,

First time posting, but have been lurking recently.

I am feeling kind of let down my urologist. Back in 11/2014, I went to the urologist to make sure my sperm was adequate to make a baby. He ran bloods and semen test and said yes, have a nice day. I trusted him fully. BTW, we still have no baby as it could be the fact I have the libido of a 100 turtle (I assume that is not good). Fast forward to today....I am feeling like crap (no libido, erections not holding, anxiety, no energy, foggy, cannot get good gains in the gym no matter how good I train and eat). I always felt like this though, but thought it may be due to an SSRI, perhaps it is, but maybe there are more variables like my T. I requested all my med records from my doctors to have a look myself. One glaring issue I see is that my Testosterone, Serum is at 267 ng/dL (at age 30; I am approaching 33 now). The doctor never said a word about it - should he have? From what I am reading, yes, but also I am reading that many healthcare professionals are not comfortable with hormones at all. I am halfway through Nelsons book on Amazon (ordered today).

Here are the results for blood ran in 2014:

Testosterone, Serum: 267 ng/dL Range: 348 - 1197
Luteinizing Hormone(LH), LH 5.8 mIU/mL Range: 1.7 - 8.6 01
FSH, Serum FSH 7.0 mIU/mL Range: 1.5 - 12.4 01
Prolactin 6.7 ng/mL Range 4.0 - 15.2

Thoughts on these numbers?

I am getting all the recommended detailed labs tomorrow AM and I will post all the results here after I receive the results. After I see the results, I am going to decide if I should engage a TRT replacement clinic and/or local provider. From what I am reading, I am not going to go through the struggle with dealing with a local provider and my insurance, just not now.

I look forward to interacting with this community. Glad I found this site.

TL;DR: 32 yo, feeling like crap (no libido, erections not holding, anxiety, no energy, foggy) and looked at medical records from nearly 2 years ago shows Testosterone Serum at 267 ng/dL ran by urologist. Now pursuing if T is still low and then therapy options if T is low.

Hey Henry - I am not freaking about having a kid like some, but if it happens, it happens. Sounds greedy, but I rather be healthy and possibly have a kid so I would want to try to keep the boys running while on TRT with HCG. If it happens, it happens. If not, thats ok. I know that I do not want to feel the way I do now.
 
Welcome to Excelmale. You need to engage a doctor who understands TRT as well as the issue of fertility. It's hard enough to find the former, finding one who understands both is an even greater challenge. You may be fortunate and connect with someone in your community who knows how the game is played...but the odds are against you. Post your results when you have them; you will certainly generate discussion.
 
If I may offer some advice..

In a young healthy male, with concerns to preserve fertility, a Stimulation Test would be warranted. This tests the functioning of your testes for 2 reasons: to identify if the testes are capable of producing enough testosterone and to see if the testes are capable but not being stimulated by the whip, LH. This is done by the use of low dose Clomid for 35 days or daily HCG injections for 30 days. At the end of the 30 day trial you have your levels checked again. Based on those results one can determine what your body is capable of doing and what's the best long term treatment for you.You still have room for improvement with your LH, which is the hormone that cracks the whip on the testes to produce testosterone. It's not a guarantee but is successful in the majority of young healthy males. Of course TRT is still an option although it is a lifelong treatment and reduces fertility. I suggest you contact Defy and schedule a consult with Dr Saya or his PA before resorting to TRT. Lastly, lifestyle variables such as diet, exercise, vitamin and supplementation, reducing stress, and sleep are major factors in testosterone levels. One extra hour of sleep can improve testosterone levels as much as 10%. These are things to keep in mind.
 
endopa-c, absolutely. Thanks for bringing that up. I will ask the Doc about this type of approach. I'd like to see if my body is capable on its own first. I've read some posts where others have found success with that regimen and others have not. I would like to be one of the ones that respond to it. ;) I am really eager to see these recent blood results first.

Here is what is coming up:

8/26/16 - called Defy, filled out all the paperwork. Ordered bloods.
8/27/16 - gave blood for all the required tests for Defy.
By 9/1/16 - I assume I should have my blood results?
9/15/16 - my annual physical with my PCP. I will then set up the consult with Defy. I expect a few week wait, so probably will occur early mid Oct?
 
Hey man that urologist needs to be punched. You paint a very strong picture of primary hypogonadism. To not tell you you're 100 points below lab range should be malpractice. He didn't tell you about an abnormal result. What's the point of seeing a doctor then?

The silver lining is that you've probably found why you feel so bad. Also you're with defy so you'll get the treatment you need. I am with defy and there's no bs involved. I'm primary and they were quick to diagnose and treat I can't recommend them enough.

Check with them on Tuesday or wendsday for your labs. They have absolutely no problem emailing them to you as soon as they receive them. Call the office and ask.

That urologist needs to have his salary set just above the poverty line of $11,000 per year, and when he complains he will be told "whats the problem, you're in the normal range of all people! Oh you don't want to be at the beginning of the normal range?"

Endopa-c I assume you're a pa based on the user name. Correct me if I'm wrong but none of that lifestyle stuff matters if a person is primary? What more can this guy do with a fairly high lh?

1) Can anyone provide me some insight on questions I should ask the doctor at my consult? / What are your thoughts on these labs?

2) 468 ng/dL is still low for my age, right? I know that is not the only marker to look at though, but that is about the extent of the depth of my understanding. :(

3) I often see SHGB in labs on here - I don't see it on mine, is this something I need for my consult?

That 267 total 2 years ago really freaked me out, maybe just a bad day.

I'm 32, 5'6", 155 lb now, body fat high teens to 20%?, don't drink or smoke, desk job, workout avg 3-4x week (I have a hell of time recovering), diet could improve some, but working on getting more consistent with that - anything else that may help to know? Thanks for taking a look.
