So I've recently started the process of enrolling with Defy medical because their telemed program fits my schedule a hell of a lot better than a traditional doctor and at this point all they've done is mess me the hell up.

I was going through my old test results trying to figure out causes and some of my options along with questions to ask when I have the conference over test results from yesterdays vampire visit. I was reviewing my old tests and noticed some things that didn't quite make sense.

Original Diag results for Test:
TESTOSTERONE TOTAL 265 ng/dL
FREE 70 pg/ml
TSH 1.5 uIU/ml
SEX HORMONE BINDING GLOBULIN 18 nmol/L
FSH 4.5 mIU/mL
LH 3.2 mIU/mL
Plus a bunch of other things that don't make a ton of sense at this point fastforward a year and the numbers get stranger.

Current as of last trough draw E5D 100mg test Cyp No AI (This doc didn't run as many pannels)
TESTOSTERONE, TOTAL, LC/MS/MS 690 ng/dL 250-1100
TESTOSTERONE, FREE 221.2 pg/mL 46.0-224.0
TESTOSTERONE,BIOAVAILABLE 464.5 ng/dL 110.0-575.0
SEX HORMONE BINDING GLOBULIN 9 nmol/L 10-50 Below low normal
ESTRADIOL 33 pg/mL
TSH 1.30 mIU/L

I've tried E3D and is what I'm currently at with the results next week and still have not found a happy balance of anything,
constant brain fog, lack of energy, and damn near 0 sex drive. Just trying to figure out what questions I should be asking aside from the basics. I've been reading a shit ton and seen guys with low SHBG not getting a ton out of TRT without low freqent doses.

When I was doing twice weekly injections SHBG was 16 and when I switched to an every-other-day protocol, SHBG increased to 22.

Low SHBG together with high normal free T usually means high free E2 which can ruin everything. My previous protocol had free T at the top of the range and E2 was always a problem.

I've seen some guys injecting daily doses who were low SHBG which saw an increase vs EOD or twice weekly.

He's right the Estrogen is the problem, likely to be the problem that is. Probably the wrong E2 testing, too. Males should only be using the (Ultra) Sensitive LC/MS/MS testing. You as a low SHBG guy should also be using "Estradiol, Free". Defy is aware of this but discuss it specifically at your appointment.
