Normal testosterone and low DHT

I am getting tested for DHT on my next blood test. I have tested normal for testosterone but still feel low test symptoms sometimes. Could it be possible I have low DHT ? Is there any cases where Guys have normal testosterone but low DHT?

We will be in a better position to help once you post your labs (including SHBG and complete thyroid labs) and indicate how long you've been on TRT. The quality of your labs will determine the quality of information you get, incomplete thyroid labs will hamper your effects.

If I'm not mistaken you have high SHBG and this can present problems which can't be ignored. It binds up testosterone, estrogen and the conversion of testosterone to DHT which doesn't happen if it's all tightly bound to SHBG.

https://jamanetwork.com/journals/jama/fullarticle/1105045 Here is a study showing that the inhibition of testosterone's conversion to dht by duasteride (5-alpha reductase inhibitor) had no significant effect on the ability of testosterone to exert its EFFECTS on muscle mass and strength, sexual function, erythropoiesis, plasma lipid levels, prostate volume, and sebum production.

He is not on trt and yes his shbg is high but his total/free t are at descent levels.

Having higher dht levels is not as critical as people think regarding the effectiveness of testosterone and its positive effects.

systemlord- I have tried vitamin D3 and stinging nettle for SHBG but it dosnt seem to be working to well. Do you know the best treatment for high SHBG? I am not on trt but i also don't want to go on trt because once I'm on you can't really come off

From one of your previous threads you stated your total t was 872 (range 242-917 ng/dl) and your free t was 22.6 (range 9.6-26.5), both your total and free t are in the upper end of the range......................no sensible doctor would put you on trt with those numbers.

If your free t was low due to your high shbg than sure trt would be worth looking into.

It is unlikely your testosterone has anything to do with your symptoms.

Thyroid, adrenals, imbalance in neurotransmitters........................there is something else going on!

Not to sound negative but your free t levels are great regardless of your high shbg so finding ways to try and lower your shbg should make no difference other than increase your free t more which you do not need as you are at the top end of the physiological range.

As I stated many times before sleep/diet (insulin insensitivity/poor diet)/excess stress whether psychological or physiological (over training gym) can all have a negative impact on ones mood/energy/libido/erections/muscle and strength gains/recovery........................it is not just simply a matter of TESTOSTERONE.

Your levels are good you need to try and pinpoint other possible factors.

Madman- I didn't think it was possible to have high free testosterone with super high SHBG. I guess you're body just sets the testosterone free when it wants too?

You've started repeated threads on DHEA, repeated threads on DHT with the exact same questions as this thread...maybe it's just "you"...have you considered that? At what point is what the membership here responds with going to be accepted? History of anxiety or OCD like behaviors here???

Lol nope no anxiety or ocd . Exact same questions? They all have all been different questions???...I don’t see anywhere where I repeated a question

Normal testosterone with low DHT and high SHBG, it's no wonder you still have symptoms. High SHBG guys need high normal testosterone to suppress overly high SHBG in order to increase FT and DHT do to it being bound up.

Question is are your FT and DHT at a level that's optimal for you. When you start TRT you will need larger injections.

Low DHT has not been confirmed yet I still need to get that tested. I’m getting DHT tested this week. My last blood test showed very high SHBG, but even though my SHBG was high my free test was still high and so was my total test.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
