Need Advice on My High Reverse T3

I am hoping that some of you with lots of knowledge relating to the thyroid, and specifically, Reverse T3 will be able to provide me some guidance. It was time for my normal blood work and I thought I would add a complete thyroid panel because I had never had it run before. I don't feel bad and was somewhat surprised to learn my Reverse t3 was so high. You'll notice on my blood work attached from July 20th that my Reverse T3 is extremely high at 26.3. This gives me a FT3:RT3 ratio of 13.96 and it should be >20. Also, my TPOab antibodies are at 25. I’ve read that TPO antibodies at that level are of no concern but not sure if that is accurate. My Free T4 at 1.34 and Free T3 at 3.70 are near what I've read to be near the optimal range, but we all know how "ranges" are.

My iron and ferritin levels are finally starting to come up now because I am supplementing. Over the years I have donated a tremendous amount of blood before learning the impact it had on my ferritin. I donated at times to control hematocrit but mostly just to donate because I thought it was a good thing to do. So what you see on the blood work results is an improvement over where I have been.

I am pooling for some reason. Could it simply be the low iron and ferritin I have been experiencing creating this? I have read that low ferritin can increase Reverse T3. Even if that is so, would it increase it that much? Will continued supplementation of iron decrease the Reverse T3 as iron and ferritin levels rise?

I am trying to do some research on RestartMed, The Tired Thyroid, and Stop the Thyroid Madness websites but am hoping for some of my fellow forum members with a lot of thyroid knowledge to give me their thoughts on these thyroid results.

My most current lab results as of 7/20/18, LabCorp, are attached.

I am also on TRT and have been for over 13 years. I am 60 years old, 5’10”, 190 lbs, 10% body fat. I work out three to four times a week, sometimes five, and feel pretty feisty. I feel healthy and I look healthy. My TRT protocol is as follows:
160 mg E3.5 days (Monday AM and Thursday PM)
500 IU of HCG on injection days (1,000 IU weekly total)
25 mg DHEA each night at bedtime
25 mg Pregnenolone each night at bedtime
No AI
No finasteride
No other medications but do partake in a few supplements.

I inject IM in the ventrogluteal (VG) most of the time but also rotate to the deltoid and vastus lateralis (outer thigh). I inject the HCG separately SubQ in the abdomen.

My T levels are over range but when I lower the dose the levels drop significantly and I don't feel quite as good, so I think I have my sweet spot. E2 is over range just a bit but no symptoms so I am okay with it. My DHT is over range but I’m not too concerned and I don’t want to block it. I feel really good on my protocol for a 60 year old.

Based in what I have read, I am surprised I don't feel absolutely crappy with such a high Reverse T3 but I don't. I am now hoping if I can flush this high Reverse T3 that I can feel even better.

Excess Reverse T3 is a growing problem for thyroid patients. It can be caused from chronic adrenal dysfunction (too high cortisol; too low cortisol) as well as having low ferritin/iron problems. Even low B12 and other imbalances, plus chronic stress, can cause the excess. You can read about the RT3 problem here.

I been reading three main websites: Stop the Thyroid Madness, RestartMed, and The Tired Thyroid trying to better understand Reverse T3. I haven't had a saliva cortisol test in about two years and at that time cortisol levels were good. However, I had never tested iron and ferritin until the past year but I have been donating blood for many,many years. My iron and ferritin have likely been very low for a very long time. They are still low but coming up.

Good thing I got my low T fixed, tweaked, and steady or I'd really be in mess.
 
Would there be any benefit by supplementing with selenium, tyrosine, or iodine?
 
You could just be asymptomatic or normal for you situation, I say that about myself on Thyroid. I lean on not subscribing to this Hypo Thyroid madness that seems to be gripping EM lately and this is a particular demonstration of it...no problems except the numbers on a page. Yet being suggested NDT, and iron and Ferritin, selenium, Iodine, etc etc...trying to be helpful but in just about every other situation, like Estradiol, you get the opposite...don't treat the number. It's funny, I think, seeing the disparity that is going on here with Thyroid.
 
You have a point. I have no symptoms, at least none that I am aware of, and I feel really good although that Reverse T3 is very high. Maybe the TRT being pretty well dialed in is compensating for how I might feel otherwise. I know my iron and ferritin have been low for a long time, probably as long as I have been on TRT, 13 years. Maybe I fix the iron/ferritin levels and all is fine?
