Lab results pre TRT -- thoughts?

48 yo male. I did the Discounted Labs ED panel, because it included the key TRT tests and ED has been a problem, so I am particularly interested in that. Thoughts? Any suggestions on a TRT protocol? Specific question: the high Prolactin. Should I treat that, or would it be addressed in a generic TRT protocol?

Dramatically hypogonadal, but somewhat depends on time of day of your blood draw. What time was blood drawn?

And I don't see LH and FSH which could help determine primary vs secondary hypogonadism. If secondary you might have other threatment options.

You would benefit from having SHBG tested to base dose and frequency.

Options for TRT range from gels to injections. Would be best to see a good Dr to sort out all these details.

E2 and prolactin are high especially realative to your T levels, so it is possible you would be prescribed an aromatase inhibitor along with initial testosterone therapy.
 
I dont see the point, yet, in picking the numbers apart, I think it's more advisable to revisit things once youre on a treatment program and see where things wash out in 90 days.

Sex/orgasm/etc etc can influence Prolactin, if you didn't abstain for 24hrs prior to your test, you should do that before you run it again. Very good point about missing LH/FSH and SHBG
 
Even though your blood draw was midday, levels are still far too low and you shouldn't be seeing those Total T numbers till much later in the day if at all. SHBG must be midrange or better do to your Free T numbers, this is good news. You will need an AI to manage estrogen while on TRT.

Men who are overweight will convert more of their testosterone into estrogen until the weight is lost.
